Oak Lawn Woman Accused Of Using Fake 'Maderna' Card In Hawaii

The 24-year-old woman presented a falsified vaccine card with the name "Moderna" misspelled to screeners in Honolulu, authorities said.

HONOLULU — An Oak Lawn woman is accused of using a fake COVID-19 vaccination card to bypass Hawaii’s 10-day mandatory quarantine for unvaccinated visitors, authorities said. Chloe Mrozak, 24, is facing misdemeanor charges with falsifying documents. The Department of Attorney General-Hawaii said Mrozak was arrested Saturday at the Daniel K. Inouye International Airport in Honolulu.

According to court documents obtained by Hawaii News Now, authorities received a tip about an Illinois visitor, later identified as Mrozak, who apparently presented a CDC COVID-19 vaccine passport with the name Moderna misspelled on the card, which proved to be a red flag for Safe Travels Hawaii airport screeners.

Mrozak arrived at the airport on Aug. 23, but before her hotel reservations could be confirmed by airport screeners, she left for her connecting flight to Oahu, authorities said. Court documents listed her place of stay as the Holiday Inn Express in Waikiki on her Safe Travels Hawaii travel itinerary, which is required for visitors to the state. When authorities checked the hotel, there were no reservations under her name. State investigators said Mrozak also left out her flight information on the itinerary.

Court documents

Investigators were unable to reach Mrozak by the phone number and email she provided. Mrozak’s CDC vaccination card indicated that she had received in her vaccine from the Delaware National Guard, but there was no record of her getting her shots there, court documents said.

An image of the handwritten vaccine card included on court documents showed the name “Moderna” misspelled as “Maderna.” The names of the Delaware guard members said to have given the shots on March 24 and April 30 were “cpl wolf” and “ssgt montey.”

Mrozak was picked up Saturday at the Southwest Airlines ticket counter at Daniel K. Inouye International Airport in Honolulu. She was traveling with another woman.

Upon her arrest, authorities said that Mrozak made a “spontaneous utter” that she had received her shots at the doctor’s officer and paid for them.

She was brought to the Honolulu Police Department, where she was processed and charged. Mrozak was unable to post $2,000 bond and is currently being held at the Oahu Community Correctional Center. She is due back in court Wednesday in Hawaii.

A Florida couple was arrested in early August for the same offense, along with a father and son from the mainland a few days prior, Hawaii News Now reported.
