Physicians at Edward Medical Group’s offices in Oswego, Sandwich and Yorkville are offering sports physicals for the upcoming school year. Appointments are required. The cost is $30.
Appointments are available between 5 – 6:45 p.m. at these locations on the following dates:

Payment (cash, check or credit card) is due at time of service. Physicals will not be billed to insurance. IHSA forms with the patient portion completed and parent/guardian signature should be brought to the appointment.
