Heritage Pointe of Fort Wayne Achieves Deficiency-Free Survey

Heritage Pointe of Fort Wayne has earned a deficiency-free survey, the highest rating a senior living community can achieve in the state of Indiana and a distinction awarded to only a small percentage of providers.
The unannounced annual inspection by the Indiana Department of Health evaluates all aspects of operations, including clinical records, quality of care, safety and cleanliness, staffing levels, and interviews with residents, families, and team members. Heritage Pointe achieved full compliance with all state and federal regulations.
The community has also received a five-star rating from the Centers for Medicare and Medicaid Services (CMS). This is the highest rating that can be received by CMS.

CEO Chod Boxell praised the team, noting the recognition reflects a culture of readiness, accountability, and commitment.
Executive Director Rod Craft added that the results affirm the team’s focus on doing what is best for residents every day. “The recognition highlights their commitment to excellent care and provides reassurance to residents and families,” he said.
