  • Make check payable to Simplistic Wholistic and mail to:  Holistic Maryland Attn: Gina Rieg/Simplistic Wholistic, 10440 Shaker Drive, Suite 103, Columbia, MD 21046.  **YOU MUST EMAIL ME to let me know that you are sending a check.  Also, with your check, you must indicate the class DATE and TIME for which you are registering, the name of the attendees, phone# to contact, email address and how you heard about the class.  Checks will be accepted only up until two weeks prior to class date.
