will be providing COVID vaccines
Date: Monday, January 10th from 2:00 PM to 4:00 PM

Registration: To assure adequate supplies for everyone attending registration for the COVID vaccine is required through the following website: https://mbrhccovidvax.timetap.com/
At the Clinic:
- Please bring insurance cards to the clinic for everyone that has insurance. The vaccine is free of charge.
- A parent/guardian is required to be present with their child.
- Dr. Frank or Dr. Cook will be available at the clinic to address any COVID vaccine related questions.
- Both the Moderna COVID Vaccine/Booster for adults ages 18 and up as well as the Pfizer COVID Vaccine/Booster for ages 12 and up will be available. Pfizer vaccine for ages 5 to 11 will also be available.
