High NCQA Ranking for Amityville Health Center

The HRHCare Maxine S. Postal Tri-Community Health Center at Amityville has been named a Level 3 Patient-Centered Medical Home.

HRHCare, a nonprofit Federally Qualified Health Center providing medical services for underserved communities throughout the Hudson Valley and Long Island, is proud to announce that the HRHCare Maxine S. Postal Tri-Community Health Center at Amityville has been named as a Level 3 Patient-Centered Medical Home (PCMH) by the National Committee for Quality Assurance (NCQA). The highest PCMH ranking available, this recognition solidifies this Health Center, along with the 21 other HRHCare Health Centers that have achieved Level 3 PCMH status, as a medical home that emphasizes care coordination and communication to transform primary care into what patients need and want it to be.

Saving the nation $24 billion annually and bringing $26.5 billion in economic activity to the communities they serve, 59% of Community Health Centers have been recognized by the NCQA as Patient-Centered Medical Homes. This is directly tied to the Community Health Center model of affordable, comprehensive care; integrating medical, dental and behavioral health care along with other services to facilitate health care access and improve health outcomes. Achieving this high level recognition depends on compliance with all of the must-pass PCMH elements; including data-proven standards for patient access and communication, implementation of evidence-based guidelines for chronic or other important conditions, active support of patient self-management and systematic tracking of test results and clinical referrals.
