New York State has issued a Missing Vulnerable Adult Alert for a Long Island man. Loderick Smith, 81, has dementia and diabetes and may be in need of medical attention.

Police said he was last seen on West Fulton Street in Long Beach, Nassau County at 11:45 a.m. Monday, Dec. 4

Smith was driving a 2016 blue Volkswagen Tiguan with New York registration EMC3765.

Similar vehicle to the one Smith is driving.
He is a black man, who is 5 feet, 10 inches tall and weighs 160 pounds. He has brown eyes and is bald.

Smith was last seen wearing a light colored pullover sweatshirt, a dark colored jacket, a black golf hat and black sneakers.
