SCI Phoenix, Pennsylvania's costliest public structure at $400 million, officially opened to inmates this summer.

The death is the first reported since the new building opened, and the first since a string of deaths earlier in the year as part of what was later called a "suicide cluster" at Graterford which left five inmates dead.

In the midst of the spree, Graterford Superintendent Cynthia Link retired. Tammy Ferguson has taken over as head of SCI Phoenix.

Pennsylvania State Police are leading the investigation into Toland's death on Sunday, as they do with every inmate death.
