Duties Ignored At Roxborough Center Where McMcaster Died: Report

ROXBOROUGH, PHILADELPHIA – After a contractor at Cathedral Village in Roxborough was charged in the death of H.R. McMcaster Sr., father of former Trump Administration National Security Advisor H.R. McMaster, the Pennsylvania Department of Health is alleging the center's staff did not conduct checks on McMaster after he fell several other times before the final fatal fall.

The report, issued Saturday, June 2 on the department's website, says McMaster fell four other times during a five-day recovery stay at the facility.

And after each of those falls, staff did not follow protocol to ensure McMaster was healthy, according to the 141-page report.

Additionally, the report accuses the center of failing to develop a fall prevention plan and alleged no one attempted to resuscitate McMaster.

The report claims the Cathedral Village administrator and its director of nursing "failed to fulfill the essential duties and responsibilities" regarding McMaster's falls.

"The facility failed to provide adequate supervision and implement interventions to prevent accidents for one resident resulting in actual harm of blunt force trauma following a fall and placed the other residents at risk for potential harm creating an Immediate Jeopardy situation," the report summary reads.

The report offers corrective measures Cathedral Village should take to address the 12 violations state investigators found at the facility.

The facility issued a statement on the document over the weekend.

"While we have been asked not to discuss specific details due to the ongoing prosecution of an outside agency nurse, what we can tell you is that there is no higher priority at Cathedral Village than the safety and health of every resident for whom we care."

McMaster, 84, died on April 13 after falling and hitting his head on April 12.

On May 10, Christann Shyvin Gainey, 30, of Philadelphia, was charged with felony neglect, involuntary manslaughter and falsifying records in connection with the death.

Gainey was working at the center as a contracted licensed practical nurse at the time of McMaster's death, which was ruled blunt force impact trauma to the head.

State authorities accused Gainey of leaving McMaster unattended in the facility's lobby area for hours after his fall and allege she did not conduct the prescribed neurological checks for fall victims and lied about completing paperwork on the fall.
