Harlee Manor Inspection Report

Harlee Manor is a Springfield nursing home that has been family owned and operated since 1968.

The Department of Health and Human Services Centers for Medicare and Medicaid Services filed a survey report on Harlee Manor on Sept. 9, 2011. 

Harlee Manor was given grades of D and F, based on three situations, on a scale from A to L, with A being the least serious and L being the most serious. 

A severity score of D through F means no actual harm was done, but there is the potential for minimal harm that does not impose immediate jeopardy, according to Medicare. 

Report Findings

According to the report filed, based on clinical record review and staff interview it was determined that the facility failed to implement the plan of care related to safe transfer for one of 16 residents reviewed. The nursing staff failed to follow the plan of care that indicated two person transfer, according to the report, and  this was confirmed by the Director of Nursing. 

Page two of the report stated, "that the facility failed to insure that alternative interventions were attempted prior to the administration of as needed hypnotics and anti-anxiety medications for two of 16 residents reviewed."

"There was no evidence that nonpharmacologic interventions were attempted prior to the use of medications on these occasions."

Page three of the report said "that the facility failed to maintain proper food storage practices and and to maintain proper sanitizing practices for the dish machine. "
