Come get all the paperwork out of the way before the school year starts.

Meet some of the coaches, the school nurse, and Athletic Director.

You will need to fill out the following forms that are

on the school web site or in the office;

A) Physical Form (physical with in the last year)

B) Sports Pre Participation Medical History (2 pages)

C) Assumption of Risk Form     

 Soccer, Cross Country, Basketball, Cheerleading, Wrestling, Softball, Baseball, Track and Field
