An attorney says the failures suggest long-standing problems, while TriStar's Nashville hospitals report using safer pre-packaged vials, and regulators and the hospital remain tight-lipped about current safety fixes.
   

2. Man arrested after reportedly waving gun outside Nashville bar (wsmv.com) — Metro Nashville police arrested a 25-year-old man after he allegedly waved a BB gun outside Brewhouse West on Charlotte Pike, following an offer from another person to call him an Uber. Officers say the man retrieved the gun from his car and pointed it at the victim's face. He now faces an aggravated assault with a deadly weapon charge and was booked on a $7,500 bond.
   

3. The death of a 5-year-old boy in Antioch is renewing urgent calls for safe gun storage (newschannel5.com) — A 5-year-old boy died after finding an unsecured gun in his Antioch home, prompting renewed pleas in Nashville for safer firearm storage around children. Community advocate Clemmie Greenlee is urging local gun owners to use locks and better hiding places, while Metro police continue investigating and offer free gun locks at all precincts.
   

4. TBI issues silver alert for woman missing out of Davidson County (wsmv.com) — Authorities in Nashville have issued a Silver Alert for 61-year-old Teresa Thomas, last seen Monday near Elm Hill Pike in Davidson County and believed to be on foot with a medical condition. Residents are urged to call Metro Nashville Police or TBI immediately if they see her or have any information about her whereabouts.
   

5. On International Overdose Awareness Day, a Tennessee family honors son lost to fentanyl by speaking out (newschannel5.com) — A Nashville family is turning their grief into action by sharing their son Quintenn's story and funding treatment and recovery through the Quintenn Clark Foundation. On International Overdose Awareness Day, they're urging Middle Tennessee families to talk openly about fentanyl and addiction, and inviting supporters to their Sept. 12 Recovery Quest fundraiser in nearby Ashland City.
