Ashburn Health Inspections: Mouse Droppings, Fruit Flies, Cockroaches

The Loudoun Health Department inspected facilities around Ashburn, Dulles, and Sterling, including a place with mice and cockroaches.

ASHBURN, VA— The Loudoun County Health Department inspected several locations around Ashburn, Dulles, and Sterling during recent inspections. Officials conducted the most recently published round of inspections between April 27 and May 3.

Many COVID-19 restrictions around the state have relaxed, so health inspectors are returning to conduct some restaurant inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.

Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.

      • A gallon of skim milk (4/22/2022) and a gallon of 2% (4/25/2022) were found in the reach in cooler at the beverage station with expired dates.
      • A container of raw chicken and a container of raw salmon were found stored on the floor of the walk-in cooler.
      • 2 spatulas were found with gouges/chips/extensive wear and tear on them.

      • The person in charge failed to describe the symptoms associated with diseases that are transmissible through food.
      • Cook observed touching the cooked naan from the tandoor with bare hands.
      • Handwashing facility observed without paper towel in the handwashing facility near the stoves in kitchen.
      • Raw marinated chicken observed stored above mixed vegetables and cooked chicken in the walk in refrigerator and in low boy refrigerator in kitchen.
      • Chemical dishwasher observed not sanitizing in that the sanitizer test trip did not change color after the wash, rinse and sanitize cycle.
      • Employees' medicines observed located located on shelf with dry food storage in the kitchen.
      • Chlorine sanitizer level registered above 200 ppm in the three compartment sink in kitchen.

      • Observed 2 Hand Sinks to not have an accessible method of drying hands after washing with water and soap.
      • Observed the following items out of temp in the Walk In: Ground Beef labeled 4/25 at 45F, Beef Steak labeled 4/26 at 45F, and Pulled Chicken at 45F.
      • Food-Consumption of Raw or Undercooked Animal Foods. A review of the menu and operation with the food service operator indicates that “salmon” may be served undercooked or raw.

      • Several live fruitflies observed on the walls and ceiling in bar area.

      • Facility did not have written procedures informing staff how to clean up a vomiting or diarrheal event.
      • Observed employee drinking container stored above food prep area
      • A hand sink in the kitchen did not have any soap to wash hands with.
      • No paper towels at hand sink located at the bar.
      • Milk observed in refrigeration unit with a discard date of April 24.
      • Pests were/was noted present in the establishment during time of inspection.

      • Chicken tagine (chicken stew) and demi-glace for pork gravy registered between 52°F to 55°F walk in refrigeration unit. As per the person in charge (PIC) said food had been cooling for more than 12 hours in the walk in refrigerator.
      • The prepared ready-to-eat (RTE) ramen broth was held more than 7 days in the refrigerator.

      • Person in charge unaware of food employees reporting agreement (Form 1B) and unable to provide the form 1B that are reviewed and signed by food employees.
      • The food establishment does not have written procedures for cleaning up vomiting or diarrheal events that happen in the facility.

      • Accumulation of standing water observed in several places throughout the facility. Stagnant water puddle on floor near soda storage area. Also stagnant water above the drain near three compartment sink. As per the person in charge (PIC) said drain is not slow to drain the water.

      • Proper oversight of food employees and food handling are not being done. Temperature abuse and improper storage of foods due to a lack of supervision was observed within the facility.
      • Raw salmon was found stored over ready to eat lasagna in the walk-in cooler, raw shell eggs were observed stored over heavy cream and pancake mix in the low boy cooler on the line.
      • Marinara and meat sauces were found at improper temperature in the steam table. Employees are taking sauces directly from the walk-in cooler and placing them in the steam table, without reheating to a proper temperature.
      • Marinara and meat sauces were found hot holding at improper temperatures (48F, 56F, 83F)
      • Pre cooked chicken wings, deli meats, heavy cream and shell eggs were observed cold holding at improper temperature (52F).
      • Numerous food items in the walk-in cooler do not have proper date marking on their containers.
      • 2 spray bottles with liquid cleaners were found without labels.
      • A large package of raw ground beef was found thawing at room temperature in a shallow metal pan.

      • No handwashing signage provided in either the men's or women's restrooms, or at the bar hand sink
      • No documentation provided that states that fish (salmon, snapper, ahi tuna) which may be served raw or undercooked have undergone parasite destruction OR have been farm raised and pellet fed.
      • Food items cold holding at the service station by the entrance to the hot kitchen cold holding at improper temperatures.

Large spill of beer in keg cabinet nearest entrance to storage area of the bar. Accumulation of trash on the floor in the service area outside of the hot kitchen. Accumulation of grease, dirt, and food residue underneath the dish machine.

      • Observed Handsink to contain one pan and 2 hoses used to fill pho cooking pot with water.
      • Observed Large Pan of Pho Broth stored uncovered underneath handwashing sink, as well as other items stored in the Walk in less than 6'' off of the ground.
      • Observed 2 in use tongs stored on oven handle pressing against oven surface.

      • Employee was observed handling dirty pans and placing them in the dish machine and putting the clean pans away without washing hands.
      • A approximately 3 pound sealed tub of tuna salad was observed cooling in the walk-in cooler with an internal temperature of 58 degrees F.

      • A container of tuna salad was observed with a date made of 4/23 while stored in prep cooler on the make line.
      • A closed container of diced tomatoes was observed cooling in the salad prep cooler with an internal temperature of 44 F.

    • Total violations: 2
      • Raw burger patties (46F) are cold holding at improper temperatures on the line at the reach in cooler.
      • Several TCS (time/temperature control for safety) foods are held under time control on the serve line ( chicken nuggets, chicken filets, sliced tomatoes, cut lettuce). Said TCS food items are not marked with time stamps nor did food employees press the automated timers for the hot food items.
