Ashburn Health Inspections: Phorever, Casa Tequila, Blend Coffee Bar
The Loudoun County Health Department recorded violations at several facilities around the Ashburn area during recent inspections.
ASHBURN, VA — Several facilities in the Ashburn area recorded violations during health department inspections between Jan. 25 and Jan. 31.
Most COVID-19 restrictions around the state have relaxed, so health inspectors are returning to conduct some restaurant inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.
Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.

- Kitchen manager could not locate proper documentation of an employee health policy showing kitchen staff are properly trained to report symptoms of illnesses transmittable through food. Manager must document staff training.

- Observed the ambient air thermometer inside of the Glass Slide "Universal Coolers" to be broken.
- Glue trap for bugs in the back of house was observed to be completely full.
- Observed Rice Scoop stored in stagnant water in container.
- Mgmt indicated that the Walk In Unit had begun to make noise and generated an electrical mildly smoky smell, so unit was turned off at 1:30 PM prior to inspector arrival. Foods were relocated to the Walk In Freezer and other Cold Holding Units.
- Observed Ice Machine’s Guard inside machine not clean and has black and red residue.
- The walls surrounding the fire suppression system contain a large accumulation of grease.

- The only handwashing facility in the facility observed blocked with boxes of food (croissant and muffins). As per the Person in charge (PIC), they just received the deliveries.

- Hand sink in the kitchen doesn't have the hand washing signage. A sign or poster that notifies food employees to wash their hands should be provided at all hand washing sinks used by food employees.
- Milk observed on the counter without proper labeling for the time. Time control food items should have a label with the time and discarded after 4 hours.
- Wet towel observed on the counter. Wet towels should be stored in the sanitizer buckets.
- Inside of the ice machine observed with accumulation of black residues. Nonfood contact surfaces of the equipment should be cleaned frequently.

- The ready-to-eat food taken from a commercially processed, hermetically sealed container was not reheated to the proper temperature for hot holding in that meatballs in marinara sauce registered at 128°F after reheating and placing them immediately in hot holding unit in front food prep/service area.
- Time/temperature control for safety (TCS) foods must be held at 41°F or below in that chicken, turkey, ham, cut lettuce, tuna salad and cheese registered between 43°F-46°F in the top section of sandwich prep cooler refrigeration unit in front food prep area. As per the PIC the unit was turned on few minutes before placing said food items. Said unit not adequately cooled to maintain the TCS food items temperature at or below 41°F.

- Raw poultry must be cooked to an internal temperature of 165°F for 15 seconds before it may be offered for consumption. The raw chicken was not cooked to a sufficient temperature and time to eliminate pathogenic bacteria in that during the inspection the cook stated that the chicken being cooked on grill was fully cooked and ready to be moved to hot holding. Said chicken registered at 157°F in the thick center.

- The establishment was not able to provide a procedure or policy to properly clean up vomit or diarrheal events. A written procedure that employees may follow to properly clean & disinfect any/all vomit or diarrheal incidents shall be provided. EHS printed NOROVIRUS clean-up worksheet and gave to the person in charge (PIC).
- The low temperature dish machine located behind the bar is not dispensing sanitizer, therefore glassware is not being sanitized after washing. Food contact surfaces shall be sanitized before use, after cleaning. PIC was able to make repair during the inspection. Sanitizer is now at 50ppm Chlorine. Corrected.
- Employee items (sweatshirt & Jacket) was observed stored with items in dry storage. Food/food contact surfaces shall be protected from sources of contamination by separating them from employee's personal items. Employee items shall be stored away from food, food contacts, prep, cook, and cleaning areas. PIC moved the employee items to the office.
- There are no testing strips to monitor the concentration of the sanitizer (Quat) at the three basin sink. Testing strips or other device that monitors the concentration of sanitizers shall be provided. Provide testing strips for Quat sanitizer.

- Observed the blade of slicer with food residue on the blade food contact surfaces should be clean to sight and touch.
- Observed improper cold holding temperature of cooked chicken 47F, 48F in walk in cooler food held cold should be 41F or below.
- Observed cutting broad at prep cooler with heavy scratching and scarring not making the cutting broads easily cleanable.

- A container of ham salad was found in a reach-in cooler located at the salad station with a date marking of 1/20/2023. This date of preparation has surpassed the 7 day window of holding. Foods that are time/temperature for safety (TCS) and are ready-to-eat (RTE) may be kept for a period of 7 days, with the date that they were prepared/opened to count as day one. Person in charge (PIC) voluntarily discarded the ham salad.

- The high temperature dish machine was unable to rise to an appropriate sanitizing temperature during the inspection (147.2F). Food contact surfaces shall be sanitized before use, after cleaning. High Temperature dish machine must reach a minimum of 160F to sanitize surfaces. Do not utilize this dish machine until it is capable of reaching 160F consistently. All utensils/equipment and food contact surfaces to be taken to the main kitchen to be washed/sanitized before use.

- Observed 4 hand sinks throughout the establishment used to store various items including, but not limited to: a soap stone, a blender full of mango smoothie, personal items, utensils, etc.
- Observed the water feeder on top of the ice bin to be exposed and contain a mold like substance throughout in contact with the water that is then fed into the ice bin.
- Observed the Everest 2 DR Prep Top to not contain an ambient air thermometer.
- Observed ice scoop in ice bin laying face down.
- Observed the High Temp Dishwashing Machine to not have a verifiable means of confirming temperature (heat registering tape, dataplate, etc.)

- The establishment was unable to provide documentation that employees have been trained in the signs and symptoms of food borne illness. Establishments must have verifiable documentation that employees have been trained in the signs and symptoms of food borne illness and how to properly report them. EHS printed a copy of form 1B and gave to person in charge (PIC). PIC made copies and had all employees read and sign form 1B during the inspection. Keep this documentation on premise at all times and available for review.
- EHS observed food debris indicative of dumping waste in the hand sink located to the left of the cooking line. Hand sinks are to be used for no other purpose other than washing hands. Do not use hand sinks for any other purpose - No dumping of liquids.
- Raw chicken was observed stored over Cooked ready-to-eat (RTE) foods in the walk-in cooler and the reach-in cooler on the line. Foods shall be protected from cross-contamination. Raw proteins shall be separated from RTE foods and arranged in a way that cross-contamination is mitigated. PIC moved the raw marinating chicken to the bottom shelf of the walk-in and reach-in coolers, below the RTE foods.
- 2 gallons of opened milk containers were found in a reach-in cooler above (44F/45F) required cold holding temperatures (41F or less). Time/Temperature control for safety foods (TCS) that are RTE shall be maintained at 41F or less. PIC moved the milk jugs to the walk-in cooler so that they may rapidly cool. EHS recommends a new thermometer for this cooler and that staff monitor the temperature to ensure that the cooler is properly holding correct temperatures before resuming storing TCS/RTE foods.
- EHS observed several cooked dishes without date marking. When keeping TCS/RTE foods longer than 24 hours, a date that the food item was opened/prepared or a discard date not to exceed 7 days from the day that the food item was opened/prepared must be labeled on the container. Date mark these items.

- Observed an aluminum pan, cup of chocolate candy and box of raisins stored in the hand sink on the line at the pantry station. Hand sink is blocked and not easily accessible for employees to properly use. All hand sinks shall be open for easy access and use so employees can properly wash their hands.
- The hand sink at the bar is not properly labeled with a sign reminding food employees to wash their hands. All hand sinks shall be labeled with a sign or poster reminding food employees to wash their hands.
- Observed TCS (time temperature control for safety) Foods (chick peas, cut lettuce, cut tomatoes) at improper cold holding temperatures in the prep cooler on the line.
- Facility does not have test strips to test the sanitizer at the low temperature dish machine at the bar. Facility should have sanitizer test strips available to monitor the sanitizer concentration of the dish machine.

- Several light fixtures above the line observed without shielding. Secondary shielding was observed on some individual bulbs within the 4 light fixtures throughout the kitchen area. Light bulbs shall be shielded, coated or otherwise shatter resistant in areas where there is exposed food, clean equipment, & utensils. Cover/shield all lights ASAP.
