ASHBURN, VA — Several facilities in the Ashburn area recorded violations during health department inspections between Aug. 1 and Aug. 8.
COVID-19 restrictions around the state have relaxed, so health inspectors conduct most restaurant inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.

- Observed half cooked broccoli, rice, and scallops stored on the counter and in the refrigerator cold holding at improper temperature (60,70,46F). Time/temperature control for safety (TCS) foods must be held at 41F or below to prevent growth of harmful bacteria.
- Observed time/temperature control for safety foods (TCS) not properly dated for disposition. Foods not labeled include: cooked rice. Ready to eat TCS foods held over 24 hours must be date marked with either made on date or date the food item must be consumed not to exceed 7 days.
- Improper methods used to thaw vacuum packaged tuna fish.
- Rice scoop stored in the standing water.
- Rice containers observed to have cracks in them. Replace the containers to provide a food contact surface that is smooth and easily cleanable, free of breaks, open seams, cracks, chips, inclusions, pits, sharp internal angles, corners, and crevices to prevent build-up of food debris.

- The person in charge (PIC) was not able to produce a verifiable employee health reporting agreement for each of the food employees.
- Observed a food employee washing their hands at a three compartment sink. Hand washing shall be done only at dedicated hand washing sinks.
- Both hand sinks in the kitchen had their water shut off and were equipped with signage that stated "do not use this sink" the sinks were also covered in foil to deter food employees from using them. When I asked the person in charge where food employees wash their hands they stated at the three compartment sink or the prep sink.
- Cooked rice dish made on 8/2/2023 (previous night) measured at 48F. Noted as improperly cooled overnight prevent the growth of bacteria. Cool TCS (time temperature control for safety) foods from 135F-70F within 2 hours and from 135F-41 or less within a total of 6 hours.
- Several RTE (ready to eat) TCS foods not date marked for disposition. cooked eggplant, yellow dal, vegetable biryani, vegetable noodles, pasta dish, chole all were not date marked for disposition.
- First aid kit and supplies improperly stored on a dry storage shelf over dry foods (flour etc) and with single service items.
- Boxes of single use articles such as to- go containers, soup containers, cups etc. are improperly stored on the floor of the dining room and in the dry storage room.
- Facility does not have low temperature dish machine sanitizer test strips for properly testing the sanitizer concentration at the low temperature dish machine.
- The facility installed a residential style ice machine directly into the water supply below the three compartment sink. A proper backflow prevention device is not installed on said direct water line connection. The condensate line is also fully submerged in a hotel pan filled with water.

- Employees unable to wash hands without soap at all sinks, with two sinks blocked, and with no hot water at the dish room area hand sink.

- Two hand sinks observed blocked with containers stored inside of sink basins. Hand sinks shall be accessible at all times. Do not block.
- Observed both the dish machine at the bar and the dish machine in the kitchen dispensing low amounts of chlorine sanitizer. Bar dish machine observed at 25ppm and 0ppm at the kitchen dish machine. Proper concentration of chlorine sanitizer is 50-100ppm.
- 2 spray bottles of cleaning agents stored directly above clean drinking glasses, plastic cups and plastic tableware. Toxic materials shall be stored away from food contact items.
- Observed 3 spray bottles of cleaning agents unlabeled. Working containers of chemicals shall be properly labeled with their common name.
- Observed many fruit flies throughout kitchen.
- Grill area food handlers observed working with food without hair restraint. Provide hair restraints to protect food and food contact from unrestrained hair.
- Observed two prep table bottom shelves in disrepair. The shelving has dislodged from the legs on one side and is lying directly on the floor. The floor cannot be cleaned on this side of the tables.
- No hot water available at the dish room area hand sink, cold faucet only is functional. Hot water shall be provided for washing hands.
- Several ceiling tiles missing or in disrepair in kitchen. Ceiling tiles shall be in place to ensure ceiling service lines and pipes are not unnecessarily exposed.
- Observed the floor/wall juncture in both the men's and women's restrooms without cove molding.

- Shredded Papaya was found cold holding in the low boy cooler to the right of the opening to the kitchen area at improper temperature (46F). Time/temperature control for safety foods (TCS) must be maintained at 41F or less. Shredded papaya was moved to the front cooler to rapidly cool.
- 2 containers of shredded papaya and one container of cut mango observed without date marking. When keeping TCS foods that are ready to eat (RTE) for longer than 24 hours, the date that the food item was prepared (cut) or a date not to exceed 7 days from the time that the food item was prepared must be labeled on the food/container. Date label these containers.
- A personal cell phone was found stored on the counter of a prep cooler, and another personal cell phone was found stored with to-go containers. Food and food contact surfaces shall be protected from sources of contamination by storing personal items away from them. Cell phones were moved to an area away from prep and to-go containers.

- No hand soap was available at the hand washing sink within the kitchen. All hand sinks that are accessible by food employees must have a suitable cleanser. Person in charge (PIC) replaced soap at the hand sink.
- Raw shrimp was observed stored over ready to eat (RTE) Indian Cheese and cooked potatoes. Food shall be protected from cross-contamination by separating animal proteins from RTE foods and arranging them in a way that mitigates the risk of contamination. PIC moved the raw shrimp to the bottom shelf.
- The low temperature dish machine is not sanitizing (0ppm chlorine when tested). Food contact surfaces shall be sanitized before use, after cleaning. EHS directed that the dish machine not be used until it is operating properly and sanitizing all surfaces effectively. The three basin sink will be used to sanitize all dishes until the repair is made. EHS demonstrated how to properly set up the three basin sink, and the correct amount of sanitizer that is needed.
- No date marking of any time/temperature control for safety (TCS) foods that are ready to eat (RTE) is being performed within the facility. When keeping TCS/RTE foods longer than 24 hours, a date marking of when the food item was prepared/opened or a discard date not to exceed 7 days from the date that the food item was prepared/opened shall be labeled on the container/food. Date label all TCS/RTE items.
- Several live and dead roaches were observed throughout the facility during the inspection. Pest shall be controlled by installing pest control devices or having pest control services and by eliminating harborage conditions. Excessive food residue on walls and faces/behind equipment should be removed.
- Most containers of food found within the facility are open ie not covered. Food shall be protected from sources of contamination by covering them (except during cooling processes) so that they are not exposed to sources of dust, dirt, splashes, drips or spills. PIC covered the containers of food with lids.
- No hair restraints were observed on any employees within the facility. Food employees must have a hair restraint such as a hat or hair net that constrains hair so that it is not exposed to food or food contact surfaces. Have all employees within the kitchen use a hair restraint.
- No test strips are available to test the concentration of sanitizer for the low temperature dish machine, or chlorine for the three basin sink. Testing strips or other testing device must be available to test and monitor the concentration of sanitizer to ensure that all food contact surfaces are being properly sanitized. Procure chlorine testing strips and use accordingly.
- Moderate to severe food residue was observed on walls, equipment faces, shelving, and behind equipment. Non food contact surfaces shall be cleaned at a frequency that precludes the accumulation of residues. Clean walls, equipment, areas behind equipment and shelving ASAP.

- Two pans of lasagna (1 meat & 1 Veggie) were found within the walk-in cooler with a date marking of 7/27/2023. Time/temperature control for safety foods (TCS) that are ready to eat (RTE) may only be kept for a maximum of 7 days from the date that they are prepared/opened, with the prep day counted as day 1. PIC voluntarily discarded both pans of lasagna. Corrected.
- The facility has no method to test or monitor the concentration of Quat sanitizer used at the three basin sink to sanitize food contact surfaces of equipment. Testing strips or other testing device must be available to test the concentration of sanitizer and monitor its effectiveness. Provide Quat testing strips.

- The hand washing sink on the line was observed blocked by a large rice cooker and inaccessible to employees. All hand sinks must be easily accessible to food employees at all times with no impediments so that frequent hand washing is encouraged. The rice cooker was moved so that the hand washing sink was easily accessible. Do not block this hand sink for any reason as it is the only working hand washing sink within the kitchen.
- Raw shell eggs and raw chicken was observed stored over ready to eat (RTE) blanched broccoli. Raw animal proteins shall be separated from RTE foods and arranged in a way that prevents cross-contamination risks. Raw shell eggs and raw chicken were moved to the lower shelf, below the RTE foods.
- A food employee was observed cooking on the line with no hair restraint. Food employees must have a hair restraint such as a hat or hair net that effectively protects hair from coming into contact with food or food contact surfaces. Provide employee with a proper hair restraint while they are working within the kitchen.

- Dark soil debris was observed on the inside bin and on the deflector of the ice machine. Food contact surfaces shall be clean to sight and touch. Clean the inside of the ice machine, and the deflector at a frequency so that soil debris does not accumulate. PIC had the ice machine cleaned during the inspection.
- A container of steak rolls was observed within the walk-in cooler with a prepared date of 7/29/23, which has surpassed the 7 day holding limit. Time/temperature control for safety foods (TCS) that are ready to eat (RTE) must not be held more than 7 days from the day that the food is prepared/opened - with the prepared date counted as day 1. Person in charge (PIC) voluntarily discarded the steak rolls.
- Three spatulas were found with varying degrees of damage to their surfaces. Food contact surfaces shall be free of any chips, cracks, breaks or other imperfections that may heed sanitizing processes. PIC voluntarily discarded the three spatulas.
- The facility does not have a testing device or strips to monitor the concentration of the quat sanitizer used at the three basin sink. A testing device to monitor the concentration of sanitizer used within the facility for food contact surfaces shall be available for use at all times. Procure Quat testing strips or other method to monitor the concentration of Quat.

Said TCS foods are not being marked or otherwise identified with a time stamp to indicate the time said TCS food was removed from temperature control. TCS foods held under time control shall be clearly marked or identified to indicate the time that is four hours past the point in time when the food is removed from temperature control.

- There is no Chlorine chemical sanitizing test kit present to ensure the chemical sanitizing solution used is at the proper concentration.
