Ashburn, Sterling Restaurant Inspections: Roaches, Fuzzy Butter Spots

The Loudoun County Health Department found violations at numerous restaurants around Ashburn and Sterling, including roaches and mold.

ASHBURN, VA — Numerous restaurants in the Ashburn and Sterling area recorded violations during health department inspections between March 6 and 20.

Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.

Here are the most recent restaurant inspections in and around Ashburn and Sterling, according to the Loudoun County Health Department. Of note, Manhattan Pizza and El Mercadito Hispano II were found with roaches. Bracket Room had butter with dark fuzzy spots, and Los Toltecos of Sterling had mold in the ice bin.

Violations:

  • This facility does not have a verifiable policy/procedure for the proper clean-up of vomit or diarrheal events. EHS emailed copy of the NOROVIRUS worksheet, and will provide a hard copy to the facility.
  • EHS observed glasses and a pitcher stored in the basin of the hand sink behind the bar. Items were removed from the hand sink during the inspection.
  • Beef Au Jus (130F), nacho cheese (70F) sauce and marinara sauce (72F) all found at improper hot holding temperatures within the warming unit on the line. Marinara & Cheese Sauces were voluntarily discarded, Au Jus was properly reheated and placed back in the warmer.
  • Chicken Birria Consommé that is made within the facility was found with a date marking of 3/10. Person in charge (PIC) voluntarily discarded the consommé.
  • The southern burger on the menu does not have a consumer advisory.

La Promesa Restaurant: 20921 Davenport Dr #112 Sterling, VA 20165

  • EHS observed several stored utensils/containers with food debris on them. Utensils/containers were moved to othe three basin sink to be rewashed.
  • EHS observed a metal container with sugar inside without a label. Person in charge (PIC) labeled the container of sugar.

  • Grease trap needs to be cleaned. There is a bad odor by the three compartment sink.

Lemon Hookah Lounge: 45985 Regal Plaza #100 Sterling, VA 20165

Violations:

  • A quart of 1/2 & 1/2, a jug of whole milk, and a container of heavy cream was observed passed the printed expiration dates on the containers. Person in charge (PIC) voluntarily discarded all three containers.
  • EHS observed an aerosol can of Hot Shot insecticide within the kitchen. PIC removed the spray from the kitchen.

Mission BBQ: 45979 Denizen Plaza Sterling, VA 20165

  • No hand cleanser was available at the hand washing sink at the dish washing area.
  • EHS noted a dirty scoop stored within the basin of the hand sink on the line. Scoop was removed from the hand sink and placed at the three basin sink to be washed.

  • Sanitizer bucket solution observed to be too strong.
  • The mechanical warewashing machine was not operating in accordance with the manufacturer's specifications. Warewashing doesn't sanitize, so it's only used to wash and not sanitize.

  • Observed Open packages of butter on the top shelf of the walk in cooler with dark black fuzzy spots of them most likely due to the uncleanliness of the walk in cooler walls.
  • Food contact surface utensils in the metal shelf above the prep sink observed heavily soiled due to the base of the shelf being heavily soiled with grime and food debris.
  • Buffalo chicken (80F) tomato soup (124F) potato soup (130F) chili (129F) noted at improperly hot holding at the steam table on the line.
  • TCS foods at the left side of the prep cooler on the line are cold holding above the minimum temperature requirement of 41F. Steak 48F, lamb 52F, hummus 45F, hot dogs 51F, cut tomatoes 51F.
  • The menu is provided with a consumer advisory reminder statement regarding the increased risk of foodborne illness when consuming undercooked meats, eggs, shellfish, and fish however the disclosure (*) is missing next to each menu item offered undercooked (burger, eggs).
  • The quat sanitizer dispensed from the three compartment sink sanitizer dispenser is at a concentration greater than the maximum allowable 400ppm.
  • Reduced oxygen packaged salmon not thawed properly. Fish left in ROP packaging at refrigeration temperatures. Fish packaging is labeled to remove from package prior to thawing at refrigeration temperatures.
  • Large accumulations of black and green cylindrical fuzzy spots built up on the walk in cooler air flaps. Dark residue observed accumulated on the walls of the walk in cooler. Dark black grime spots observed on the top of the ice deflector of the ice machine.

  • Raw shell eggs improperly stored over vegetables in the walk in cooler.
  • TCS (time temperature control for safety) foods (sliced sausage 50F, crumbled sausage 49F, mozzarella cheese balls 48F) are cold holding at improper temperatures at the pizza prep unit. All TCS foods in a shallow 2 inch hotel pan are holding above the minimum 41F. TCS foods in deep pans (deeper into the air circulation of the cooler) are maintaining 41F or less.

  • Handwashing facility observed without appropriate hand drying provision in that the hand sink at the bar observed without paper towels.
  • Stored food is not protected from cross contamination from raw meat in that observed large containers of raw chicken observed being prepped/cooked placed on top of the low boy refrigerator next to the ready to eat uncovered cut lettuce, tomatoes and cheese. Said lettuce, tomatoes and cheese observed stored in open top section of the low boy refrigerator in the kitchen.
  • Grilled chicken and raw salmon registered at 46°F in two of the four drawer low boy refrigerator in front of the grill and fryer in the kitchen. As per the PIC said food had been in said unit for an hour and said unit was frequently opened.
  • Observed an ambient temperature at 52°F in the top section of low boy refrigeration unit holding raw eggs.
  • Cut leafy greens and cheese registered at 60°F in two of the four drawer low boy refrigerator in the kitchen. Said unit registered operating at 60°F. As per the PIC said food had been in said unit for an hour. Said unit was unplugged.
  • Low boy drawer refrigerators observed not maintaining time/temperature control for safety (TCS) foods at 41°F or below.
  • Hand washing sink in the coffee prep area observed without cold running water.

& Pizza (C Terminal): 1 Saarinen Cir. Dulles, VA 20166

  • Person in charge (PIC) must be aware and inform all employees of their responsibility in reporting symptoms or diagnosis of diseases transmissible through food. Manager was unable to provide a signed copies of employee health reporting agreement.
  • Open drinking containers stored in a manner that may contaminate food, food contact surfaces or utensils in that open cups of tea and coffee observed on food prep table in food prep area in the facility.
  • Handwashing facilities in the front food prep area and in kitchen observed without paper towels.
  • Dishwasher at the bar area observed not sanitizing after the wash and rinse cycle.
  • Cut arugula and basil leaves registered at 62°F in top section of pizza topping refrigerator next to the pizza oven.
  • Time/temperature control for safety (TCS) foods must be held at 41°F or below in that bags of cheese registered at 52°F in the under counter pizza prep cooler in front pizza prep area. Said section of the unit registered operating at 50°F.
  • Raw shell eggs registered at 54°F in top section of pizza topping refrigerator. Said section of the unit observed open and next to the pizza oven.
  • Several live fruitflies observed in the bar area.
  • Metal floor in the walk in refrigerator observed torn and needs repair.

Subway Concourse C: 1 Saarinen Cir. Dulles, VA 20166

  • There is no available Quaternary sanitizer at the facility for sanitization. As per the PIC the facility ran out of sanitizer.
  • Cut leafy greens, rotisserie chicken, roast beef, grilled chicken, ham, turkey and cheese registered at 45°F in open sandwich prep cooler in front food prep/service area.

The Don's Wood-Fired Pizza: 21018 Southbank St Sterling, VA 20165

Violations:

  • The food handler certificate has expired. No other old certificates available.
  • Person in charge (PIC) was only able to provide an employee health reporting policy for 4 employees at the time of the inspection. 5 employees are on shift.
  • EHS observed ice that had been dumped in a hand washing sink at the front counter.
  • A container of portioned cooked sliced chicken breast was found cold holding at improper temperature (45F) in the walk-in cooler.
  • Several containers of cooked chicken wings, chicken breast portions, cut cheeses, sausage and meatballs were observed with no date labeling. Employee date marked these items.
  • EHS observed 2 Tupperware containers with severe damage that were in use. Both containers were cracked, missing pieces and unable to be cleaned/sanitized. PIC discarded both containers.

Los Toltecos of Sterling: 21100 Dulles Town Cir #150 Sterling, VA 20166

Violations:

  • Observed a half cup of coffee/milk at warewashing area and near a prep sink.
  • Observed the far left hand sink at the bar area used to store a cup, observed the far right hand sink at the bar area used to store Tito's Vodka Alcohol Absorber Mats.
  • Observed the water feeder on top of the ice bin to be exposed and contain a mold like substance throughout in contact with the water that is then fed into the ice bin. This has continued since the previous inspection.
  • Upon entering facility inspector observed warewashing of pans being conducted in the ES 2000HT. The ES 2000HT was tested with multiple loads and at highest reached a maximum temperature of 126F utilizing a Test Plate. Dishwashing machine at bar is out of order. Observed QUAT sanitizer for 3 VAT sink not connected to the faucet dispenser. PIC was instructed how to properly sanitize dishes with bleach by instructor, which PIC had previously stated was only used for floor cleanings, then stated operator had intended to use bleach for sanitization.
  • Observed 2 working spray bottles of bleach unlabeled.
  • Observed ice scoop in ice bin lying face down (2) and two in use tongs pressed against the oven.
  • Observed that Ecolab Undercounter unit at bar is not functioning. Observed the ES2000 HT is only registering a temperature 126F when tested with a disk. Observed the Quat dispenser at the 3 VAT sink to be in a state of disrepair.

King Pollo: 22330 S. Sterling Blvd #113 Sterling, VA 20164

  • Observed 4 cups of employee beverage throughout prep areas.
  • Observed employee donning gloves twice while serving food to public without washing hands first.
  • Observed all 3 paper towel dispensers empty.
  • Observed the following items in the C5 hot holding unit to be under temp: Macaroni and Cheese at 123F and cooked rice at 127F.

  • There is no available Chlorine sanitizer in bar dish machine. Sanitizer is reading 0 parts per million.

  • Chemical spray bottle observed without the proper labeling.
  • There is a gap between the bottom of the back door and the floor.
