The SHP has partnered with ALL Pediatrics to be able to offer an in-school flu clinic on October 19. Nurses from ALL Pediatrics will be available to answer questions and hand out information and consent forms at all the Back to School nights. If you are unable to attend Back to School Night, or would like to review the information in advance, you can find it in the Forms section of this website, or it is also attached below
The cost is $25, unless you are covered by Medicaid; there is no cost for those covered by Medicaid. ALL Pediatrics will be able to accept cash payments for the vaccinations at B2S nights; otherwise, payment must be received via check to Lorton Station SHP no later than October 8. Information about the vaccine and a consent/payment form is attached below.
