Manassas Health Inspections: McDonald's, Bonefish Grill, Joe's Pizza

The Prince William Health District recorded violations at several facilities around Manassas and Gainesville during recent inspections.

MANASSAS, VA — Several facilities in the Manassas area recorded violations during recent health department inspections between March 22 and March 28.

Most COVID-19 restrictions have relaxed around the state, and health inspectors are returning to conduct many restaurant and other health inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.

Violations typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection. Other violations are addressed later, and an inspector may conduct a follow-up inspection to ensure compliance.

    • Date inspected: March 22
    • Total violations: 7
      • There was no verifiable employee health policy was not available at the time of the inspection.
      • There was no procedure/bodily fluid kit available during the inspection to show how to clean vomit and diarrhea.
      • The hand sink was blocked by a container of food.
      • The cutting boards were heavily scratched and scored, the kitchenaid stand mixer was visibly covered in food debris and not clean to the touch.
      • There were packs of Reduced oxygen packaging salmon in the reach in cooler completely thawed.
      • There were no test strips for the Quaternary Ammonium chemical sanitizer solution.
      • Mops were laying inside the mop bucket full of water not actively being used.

    • Date inspected: March 23
    • Total violations: 12
      • There were several food items and ingredients that were opened, exposed and uncovered in the facility.
      • A container of cooked mashed potatoes were hot holding at 124F.
      • The sliced tomatoes were placed in the top part of the sauté station cold holding unit after being sliced/prepped.
      • There were bulk white containers that held food ingredients without a label barring the name of the item.
      • In between use utensils were placed into containers of water that were temp at 74F.
      • There was food containers that were stacked on top of each other without them being properly dried before stacking.
      • The Eco Lab Stationary High Temp dishwasher upstairs below the conveyor dishwasher was not in good repair/not working.
      • There were no test strips for the Chlorine chemical sanitizer solution.
      • There were no irreversible test strip indicators for the high temp dish machine in the facility.
      • There was a y connector on the service sink hose in the on position while there was a backflow prevention device in place.
      • There was foil covering the piping on the 3 compartment sanitizer station to lead into the plumbing for waste.
      • The outside trash receptacles did not have a top to properly close the top.
  • Joe's Pizza & Subs, 14085 Dave’s Store Ln. Gainesville, VA 20155
    • Date inspected: March 27
    • Total violations: 14
      • There was no verifiable employee health policy was not available at the time of the inspection.
      • There was no procedure available during the inspection to show how to clean vomit and diarrhea.
      • An employee had an open container of coffee that was resting on top of the Grista 2 door low freezer.
      • There was no individual towels near the hand sink.
      • The hand sink was blocked by lettuce.
      • An employee was observed working with food without a proper hair restraint on.
      • There were single use items being reused to hold other food and ingredients.
      • The shelving in the walk in cooler was rusted, not smooth and not easily cleanable.
      • The prep table were food containers were not smooth and easily cleanable.
      • There was plastic bags that were used as stoppers to hold water in the 3 compartment sink basin.
      • The fan in the kitchen was covered in dust.
      • There was no air gap from the 3 compartment sink.
      • The walls were covered in grease and debris, also there was mold on the caulking at the hand sink near the 3 compartment sink.
      • Mops were laying inside the mop bucket full of water no actively being used.
  • Bonefish Grill #0704, 7611 Somerset Crossing Dr Gainesville, VA 20155
    • Date inspected: March 27
    • Total violations: 4
      • A bag of cooked rice was hot holding at 131F.
      • A covered container of chopped lettuce, was placed into a cold holding unit right after being prepared.
      • There were packs of Reduced oxygen packaging salmon, cod and other varieties of fish in the reach in cooler completely thawed.
      • There were no irreversible test strip indicators for the high temp dish machine in the facility.
  • GMU Hylton Performing Arts Center, 10900 University Blvd Manassas, VA 20110

    • Total violations: 1
      • The person in charge is not able to provide a verbal or written policy for preventing the transmission of foodborne disease by an employee who has a disease or a medical condition.
  • McDonald's, 8287 Shoppers Sq Manassas, VA 20111
    • Date inspected: March 28
    • Total violations: 3
      • Fries cooler was observed in a state of disrepair.
      • 2 Door cooler was not maintaining proper cold holding temperature 49 F.
      • Wet mop is not placed in a position that allows it to air-dry without soiling walls, equipment, or supplies.
  • Pizza Hut #37448, 8263 Shoppers Sq Manassas, VA 20111
    • Date inspected: March 28
    • Total violations: 3
      • The Person In Charge (PIC) was not able to provide documentation that all food employees or conditional employees were trained and informed of their responsibilities to report symptoms or diagnosis of, or exposure to, diseases transmissible through food.
      • The food establishment does not have written procedures for cleaning up vomiting or diarrheal events that happen in the facility.
