You do not pay more. Just click at the top of the smile.amazon.com page to designate your favorite charity. I put in Food for Others, but you can put in any charity you choose.
In addition, Food for Others has created a Wish List on smile.amazon.com; foods that you buy and donate to us will be shipped directly to FFO. So Food for Others gets your gift as well as 0.5% of the purchase price. Whether you are buying for yourself or buying for Food for Others, at smile.amazon.com your 0.5% donation is automatic!
