Vienna Restaurant Inspections: Bazin's on Church, China Village
Health inspectors found Vienna restaurants with as many as eight and nine violations during April inspections.
VIENNA, VA — With Fairfax County Health Department's restaurant inspection reports back online, we continue to look at reports in the Town of Vienna.
The health department's environmental health staff inspect over 5,200 restaurants and other retail food service establishments in Fairfax County and cities of Falls Church and Fairfax to ensure safe food handling practices are followed. This includes proper sanitation, food storage and preparation, and adequate kitchen facilities. Aside from restaurants, staff inspect child care centers, hospitals, school cafeterias, and temporary food service vendors at festivals.
Until summer 2022, food establishment inspection reports had been unavailable online since 2019 as the health department worked to launch a new online system. Residents can now search for a specific restaurant or browse a map of inspected locations through the online database.

The health department cautions residents that multiple inspection reports should be used to evaluate a restaurant rather than a single report.
This week's report focuses on April reports around the Town of Vienna. Nozzo Pazzo, Bazin's on Church and Lotus Garden each had nine violations, while China Village, Yama Japanese Restaurant and Hawk & Griffin each had eight violations. More details are available in each restaurant's report.

- Observed that paper towels were not provided at the hand washing sink that is located at the 3-compartment sink station.
- The food temperature measuring device used by the certified food manager is not operating properly.

- The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: tuna salad.
- The backflow or backsiphonage prevention device installed on the mop sink and the spray hose on the 3 vat sink is not identified as meeting standards set by the American Society of Sanitary Engineering.
- The chicken used for chicken salad was not cooled properly and after 6 plus hours was still temping at 50f.
- EHS observed no sanitizer being used in the facility to sanitize dishes or equipment.
- Observed the hand washing sink located to the left of the meat and cheese 2DR UPR cooler, blocked by a cart of bread and also had an empty sanitizer bucket in the sink preventing access by employees for easy handwashing.
- There is no test kit located in the facility for monitoring the concentration of the chemical sanitizing solutions.
- The following food items that are not easily identified by appearance were observed without a label: Flour, Sugar and unidentifiable spices.
- Milk crate(s) found used for the following purpose(s): as storage of dry goods, bags of flour and cooking utensils.
- EHS observed no handwashing signs posted throughout the facility.
Amma Vegetarian Kitchen: 344 Maple Ave., Vienna, Va 22180

- Observed that the floor under stationary equipment and under the front hand washing sink is in need of cleaning.
- Unwrapped knives, forks, or spoons were not stored with the handles up or facing customer side/direction.

- Raw animal food storage, preparation, holding or display conducted in a manner that may cause cross contamination of ready-to-eat food as follows: raw chicken, raw beef stored above cooked foods and vegetable inside the walk-in refrigerator.

- The certified food manager could not provide a food temperature measuring device.
- Unwrapped or uncovered food in the following location where the food is subject to contamination: processed and cleaned vegetables and animal foods were not observed covered during storage in the walk-in refrigerator.
- The following food items that are not easily identified by appearance were observed without a label:salt, sugar, msg, and other spices. Labeled during the inspection.
- The nonfood-contact surfaces of the following equipment had accumulations of grime and debris: prep tables and shelving under table, exterior surges of black refrigerator.
- Observed the following food thawing using an improper method: soaking shrimp in a bucket and stored at room temperature.
- Wet wiping cloths used for wiping counters and other equipment surfaces were observed stored on the counter.
Domino's Pizza: 331 Maple Ave., Vienna, Va 22180

- The following equipment/utensils were observed soiled to sight and touch: sauce preparation table located at the back room was soiled and in need of cleaning.The interior of the reach-in beverage refrigerator was also noted to be soiled at the base.
- Unmarked spray bottles containing toxic cleaning chemicals were not labeled to identify contents.
- Observed that the floor along the base of the wall near the grease trap is in need of cleaning.
- Food stored in a location where it is subject to splash, dust or other contamination as follows: a bag of corn flour was found stored next to a wash/dry machine in the back room where it was subjected to possible contamination.
Pizza Vienna: 235 Maple Ave., Vienna, Va 22180

- Raw marinated chicken was observed ed stored above a lettuce in the reach-in refrigerator.
- The food establishment does not have written procedures for employees to follow when responding to vomiting or diarrheal events.
- Observed that paper towels were not provided at front serving area and the back work area handwashing sinks.
- The certified food manager could not provide a food temperature measuring device.
- Observed the hand sinks throughput the food prep and serving areas were being used to store and cleaning small items.
- Observed that hand soap was not provided at back area handwashing sink.
- The operator is not testing the chemical sanitizing solution to ensure proper concentration of the solution.
- There are no chemical sanitizers for sanitizing equipment and utensils available in the establishment. CFM provided chlorox as a sanitizer during the inspection.
Smoothie King: 258 Maple Ave., Vienna, Va 22180

- An air gap between the water supply outlet and the flood rim level at the 3-compartment sink is not observed.
- Dispensing and/or in-use utensils improperly stored between use as follows: scoops placed on food with handles in direct contact with the food, on the serving table.

- There is no soap in the soap dispenser at the cook-line hand washing sink for employees to properly wash their hands.
- The following equipment/utensils were observed soiled to sight and touch: interior surfaces of the refrigerators on the cook-line were soiled and in need of cleaning.
- Observed unmarked spray bottles containing toxic cleaning chemical solutions, were not labeled top identify contents.
- The nonfood-contact surfaces of the following equipment had accumulations of grime and debris: The top surface of the drawer refrigerator which is located under the grill has excess debris and grease build-up. The exterior surfaces of the cook-line reach-in refrigerator are grease and soiled. The storage shelving under the cook-line steam table is in need of cleaning.
- Dispensing and/or in-use utensils improperly stored between use as follows: Ice scoop found laying directly on top of the ice machine.
- Observed that the floor under equipment on the cook line is in need of cleaning has debris and grease build-up.
Sushi Yoshi: 101 Church St., Vienna, Va 22180

- Restricted, commercial use pesticides present in the facility are applied by the operator who is not a certified pesticide applicator. Also, household use only pesticides are being used in the facility.
- The following equipment/utensils were observed soiled to sight and touch: interior surfaces of freezers located down stairs were soiled and in need off cleaning.
- The nonfood-contact surfaces of the following equipment had accumulations of grime and debris: The exterior surfaces of refrigeration units located in the downstairs dry storage room were noted to be very stained and in need of cleaning.
- Wet wiping cloths used for wiping counters and other equipment surfaces were observed stored on the counter.
- Observed that inadequate lighting was provided in the consumer self-service areas, reach-in refrigerators, handwashing areas, warewashing areas, equipment andstorage areas, and/or toilet rooms. Down stairs food storage room. Some of the ceiling light bulbs have burned out.
- Observed that carry plastic food container boxes were stored on the floor that exposed them to contamination.

- Observed unmarked spray bottles that contain toxic cleaning chemicals without labels to identify contents.
- Observed the following food thawing using an improper method: raw shrimp soaked in water in a container, to thaw.
- Observed that sign or poster that notifies food employees to wash their hands is not provided or is not clearly visible at all handwashing sinks used by food employees.
- Unwrapped or uncovered food in the following location where the food is subject to contamination: walk-in refrigerator.
- The following refrigeration unit is not operating properly and is unable to maintain cold food at or below 41F: MB Master refrigerated prep table on the cook-line.
- Observed that the floor under equipment in the kitchen /cook-line has grease and debris accumulations and is in need of cleaning.

- The following equipment/utensils were observed soiled to sight and touch: interior of reach-in. Refrigerators and freezers debris noted at bottom of each unit.
- The nonfood-contact surfaces of the following equipment had accumulations of grime and debris: exterior surfaces of equipment are soiled and in need ofcleaning. Shelves under the prep table at the back room have debris accumulations and is in need of cleaning.
- Observed that the floor under shelving in side the walk-in freezer and walk-in refrigerator is in need of cleaning. The wall inside the walk-in refrigerator is also inneed of cleaning. The floor under stationary equipment is also in need of cleaning.
- Unwrapped or uncovered food in the following location where the food is subject to contamination:un converted food containers were noted in. The walk-inrefrigerator and walk-in freezer.
- Food stored on the floor and/or food stored less than 6 inches off the floor inside walk-in. Freezer.
- The lid for the ice machine is detached and missing exposing the ice to possible contamination.
- The following hot display case used for holding roasted duck is not operating properly and is unable to maintain cold food at or above 135F.
Yama Japanese Restaurant: 328 Maple Ave., Vienna, Va 22180
Last inspection: April 19
- The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: Salmon=45, tuna=43, yellow tail=49F at sushi display case (left case).
- Observed food employees using improper handwashing procedures. Employee at the sushi bar was observed washing his hands with the gloves on.
- The concentration of the chlorine in solution used for soaking wet wiping towels was measured at above 200 ppm. This concentration exceeds the approvedlimits for use with food-contact surfaces of equipment and utensils in a foodservice establishment.
- Observed that hand soap was not provided at sushi bar handwashing sink.
- Observed the handsink at the sushi bar is being used for dumping and washing of small utensils.
- The following food items that are not easily identified by appearance were observed without a label: Observed unmarked cornstarch containers without labels to identify contents.

- One of the sinks at the bar is not operating properly and has been placed out of service until repairs have been completed.
