Insects Found On Kitchen Floor: Woodbridge Health Inspections

The health department found insects on kitchen floors during recent restaurant inspections in the Woodbridge area.

WOODBRIDGE, VA — Several facilities around Woodbridge recorded violations during the most recent round of health department inspections from Jan. 11 to Jan. 17, including a pair of facilities with insects in their kitchens.

Most COVID-19 restrictions have relaxed around the state, and health inspectors are returning to conduct many restaurant and other health inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.

Violations typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection. Other violations are addressed later, and an inspector may conduct a follow-up inspection to ensure compliance.

  • Occoquan Elementary School, 12915 Occoquan Rd Woodbridge, VA 22191
    • Date inspected: Jan. 11
    • Total violations: 2
      • Employees drinking from open containers (soda cans).
      • The waste lines from the 2 compartment prep sink and the 3 compartment warewashing sink in the kitchen are directly connected into the sewage system. The waste line from the ice machine is not properly air gapped either.
  • Hampton Inn, 16959 Old Stage Rd Dumfries, VA 22025
    • Date inspected: Jan. 11
    • Total violations: 4
      • There is no person in charge with a valid Food Protection Manager Certification from an accreditied agency recognized by the Conference for Food Protection.
      • Management, food employees or conditional employees were not aware or informed of their responsibilities in reporting symptoms or diagnosis of diseases transmissable through food.
      • Observed that hand soap was not provided at each handwashing sink.
      • No disposable towels were provided a the hand washing lavatory in the in kitchen.
  • Domino's Pizza, 16430 Navigation Drive Woodbridge, VA 22191
    • Date inspected: Jan. 12
    • Total violations: 3
      • The Person in charge was not able to provide documentation that all food employees were trained and informed of their responsibilities to report symptoms or diagnosis of or exposure to diseases transmissible through food.
      • This food establishment does not have written procedures for cleaning up vomiting or diarrheal events that happen in the facility.
      • Observed toxic materials (Sanitizer) was found stored or displayed close to canned food products in such a way as to contaminate items at the dry storage area.
  • Al's Buffalo Wings, 5860 Mapledale Plaza Woodbridge, VA 22193
    • Date inspected: Jan. 12
    • Total violations: 3
      • This food establishment does not have written procedures for cleaning up vomiting or diarrheal events that happen in the facility.
      • Observed Pests/insects was seen on the floor of the kitchen area at this food establishment during inspection.
      • Observed that inadequate lighting was provided in the reach in refrigerators and/or dry storage areas.
  • Dixie Bones BBQ, 13440 Occoquan Rd Woodbridge, VA 22191
    • Date inspected: Jan. 12
    • Total violations: 16
      • This food establishment has not established written procedures for responding to vomiting or diarrheal events that happen in the facility.
      • Signage that notifies food employees to wash their hands was not provided and/or permanently affixed at all handwashing sinks used by food employees (handwashing sink on service line, currently labeled "no dumping grease").
      • Flat of raw eggs stored over ready to eat pickles in walk-in cooler (WIC).
      • Spray bottle of chemicals (blue liquid, Windex, per PIC) in kitchen mislabeled sanitizer.
      • Spray bottle of chemicals stored near foods (bbq sauces).
      • Various containers of coleslaw (just made this morning at 10am) were found at 44 degrees F on the prep/reach-in refrigerator and in the walk-in cooler (WIC).
      • There was no ambient air temperature measuring device (thermometer) in the 2 door prep/reach-in refrigerator.
      • Various bulk food containers were not labeled properly with the common name of the foods stored within them.
      • The ice in the bin adjacent to the handwashing sink in the take out area is exposed to contamination when employees are washing their hands.
      • Employees handling food were observed to not be wearing proper hair restraints.
      • Ice cream scoop in kitchen was observed to be improperly stored in between usage in standing water at room temperature.
      • Single service styrofoam cups stored adjacent to the handwashing sink in the take out area are exposed to contamination from the splash of waste water .
      • Single use foil containers were washed and stored on the equipment storage shelf for re-use.
      • Cardboard being used to line shelving in walk-in cooler and along with plastic to line storage shelving near the ware washing machine.
      • The temperature measuring gauges on several of the Henny Penny hot holding cabinets are in disrepair. The temperature measuring gauges on the Meiko ware washing machine are not registering the required temperatures, even though the unit appears to be sanitizing properly (thermolabel turned black).
      • Milkcrates were being used as shelving and storage containers throughout the kitchen.
  • Cheesecake Factory, 2708 Potomac Mills Cir Woodbridge, VA 22192
    • Date inspected: Jan. 17
    • Total violations: 2
      • Employees and conditional employees are not aware of the reporting requirements concerning information about their health and activities as they relate to diseases that are transmissible through food.
      • This food establishment has not established procedures for cleaning up vomiting or diarrheal events that happen in the facility.
  • World of Omelettes & Waffles, 13775 Jefferson Davis Hwy. Woodbridge, VA 22191
    • Date inspected: Jan. 17
    • Total violations: 23
      • Signage that notifies food employees to wash their hands was not provided and/or permanently affixed at all handwashing sinks used by food employees (handwashing sink in drink/service area and handwashing sink near 3 compartment sink).
      • The handwashing sink in the drink/service area was being used to rinse equipment such as the soda machine parts found within it.
      • Peppers in the Hoshizaki upright refrigerator were observed to be discolored and moldy in appearance; hair was observed to be in the sugar.
      • Raw steak in sandwich prep refrigerator #2 stored uncovered.
      • Homemade chicken tortilla soup (made 1/9/2023) still in Hoshizaki upright refrigerator; commercially prepared mac n' cheese (Oct 2022) and hot dog buns ((Aug 2022) also in Hoshizaki upright refrigerator and out of date.
      • Bottle of soap was found stored above the soda dispenser.
      • Observed a roach by the handwashing sink along the cookline. Observed many small flies (fruit? drain?) throughout the kitchen.
      • Employees handling food were observed to not be wearing proper hair restraints.
      • Scoops for butter in service area were observed to be improperly stored in between usage in standing water at room temperature; plate being used to scoop cooked diced potatoes in container in walk-in cooler.
      • Clean equipment was stored on the shelving near the ice machine with the food contact surfaces exposed to dust and other contamination.
      • Clean plates were stored on the cutting board on the sandwich prep refrigerator with the food contact surfaces exposed to dust and contamination (visibly soiled with food debris); clean dishes were also stored near dirty dishes.
      • Single service containers throughout facility were not in their original packaging or inverted and directly exposed to possible contamination.
      • Single use whipped margarine container being reused to store foods.
      • The equipment shelving near the ice machine is in disrepair and no longer easily cleanable.
      • Cardboard is being used to line shelving; plastic is being used to line shelving inside sandwich prep refrigerators.
      • Observed a bread rack being used as shelving in the walk-in freezer.
      • The faucet at the 3 compartment sink is in disrepair (leaking).
      • The wastewater pipes of the 3 compartment sink extend down into the floor drain, eliminating the air gaps; the wastewater pipe at the ice machine does not appear to be air gapped; there are leaks in the wastewater pipes at both the 3 compartment sink as well as the ice machine.
      • The waste receptacle in the employee restroom used by the food service staff was not covered.
      • Walls, floors, ceilings, waste water lines, floor drains, etc throughout the kitchen are unclean with visible, accumulated soil. Spillage of white granular substance noted in dry storage area.
      • There is a hole in the floor along the cookline; there are tiles missing from the walls and floor throughout the kitchen; there are holes in the ceiling in the kitchen overtop of the warewashing machine.
      • Observed that there were several pieces of equipment in disrepair, unused, throughout the facility. There were also storage areas with a lot of equipment unnecessary for the operation of the facility.
      • Adequate lockers or other suitable facilities are not provided for employees clothing and other possessions resulting in personal items such as backpacks, purses being stored on shelving with food and along service line with single service items.
  • Del Campo Deli & Grill, 2780 Metro Plaza Woodbridge, VA 22192
    • Date inspected: Jan. 17
    • Total violations: 15
      • There was no verifiable employee health policy was not available at the time of the inspection
      • There was no procedure/bodily fluid kit available during the inspection to show how to clean vomit and diarrhea.
      • There was a cup of uncovered coffee sitting on the prep table cutting board
      • There was an employee that was handling ready to eat vegetables with barehands.
      • The hand washing sink that is beside the 3 compartment sink was blocked by shelving and equipment
      • There was no hand soap at the hand sink near the 3 compartment sink.
      • There were no hand towels at the hand sink near the 3 compartment sink.
      • The cutting boards at the sandwich prep table were no longer easily cleanable, heavily scratched and scored.
      • There was a container of black beans that were cooked on 1/16, temp at 47F today.
      • There was a container of paint that was above single service items. Also there was fuel directly beside plantains without space between.
      • There were aluminum pans that were turned upright that had food debris inside of them.
      • There was cardboard holding up the dishes on the drying rack above the 3 compartment sink.
      • the faucet at the 3 compartment sink was leaking.
      • There was direct connection to the sewage system from the wash and rinse basin.
      • The light insinsity under the hood system was dim and not enough light to properly be able to see while working with food.
  • Comfort Inn, 16931 Old Stage Rd Dumfries, VA 22025
    • Date inspected: Jan. 17
    • Total violations: 2
      • This food establishment does not have an employee, with management and supervisory responsibility, that is a Certified Food Protection Manager.
      • Reach in cooler broken.
