Woodbridge Health Inspections: 28 Violations At One Restaurant

Several Woodbridge facilities recorded violations during recent inspections from the Prince William County health department.

WOODBRIDGE, VA — Several facilities from eastern Prince William County recorded violations during the most recent round of health department inspections from July 19 to July 26.

COVID-19 restrictions have relaxed around the state, and health inspectors conduct most restaurant and other health inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.

Violations typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection. Other violations are addressed later, and an inspector may conduct a follow-up inspection to ensure compliance.

  • Hunan Garden, 12437 Hedges Run Dr Lake Ridge, VA 22192
    • Date inspected: July 19
    • Total violations: 28
      • The Person In Charge (PIC) was not able to provide documentation that all food employees or conditional employees were trained and informed of their responsibilities to report symptoms or diagnosis of, or exposure to, diseases transmissible through food.
      • This food establishment has not established written procedures for responding to vomiting or diarrheal events that happen in the facility.
      • Open drinking containers were stored (on the food prep table, near single service items) in a manner that may contaminate food, food contact surfaces or utensils.
      • Signage that notifies food employees to wash their hands was not provided and/or permanently affixed at all handwashing sinks used by food employees (handwashing sink in front service area).
      • Various foods in walk-in cooler and walk-in freezer stored uncovered.
      • Raw beef on a stick stored over ready to eat (RTE) cooked sweet n' sour chicken in reach-in refrigerator; raw shelled eggs stored over RTE foods in walk-in cooler.
      • Raw meats in secondary packages (not original packaging) stored adjacent to each other in the freezer.
      • Observed an employee wash and rinse equipment, but not sanitize it.
      • Slicer unclean with visible food debris.
      • Egg drop soup @ 95 degrees F being reheated on the steam table.
      • Chemicals (WD-40) stored near the slicer in the kitchen.
      • Various containers of vitamins and medicines were stored near food and food contact equipment throughout the kitchen.
      • Spray bottles of chemicals in kitchen unlabeled.
      • Various foods along cookline and other foods in bulk storage containers were not labeled properly with the common name of the food.
      • The outer opening of the food establishment is not protected against entry of insects and rodents (vermin). 1. The solid outer opening door was not closed upon arrival. 2. The screen door in front of the solid outer opening door is in disrepair with a 1" opening all around the door.
      • Rice cookers being used in front of and beside handwashing sink in kitchen area subjecting rice to possible contamination from the wastewater from the handwashing sink.
      • Food stored on the floor in the walk-in freezer; food stored on the floor near the back door.
      • Knives were observed to be stored in between equipment.
      • Various single use metal cans and plastic containers (such as cashew containers, etc) being reused to store other foods in throughout the kitchen; employee observed washing cans and plastic containers for reuse.
      • Single service styrofoam carryout containers were stored along cookline with food contact surfaces exposed to possible contamination.
      • Single service items without handles being re-used to dispense bulk food items.
      • Cardboard (flat and egg crates) being used throughout the facility to line shelving, ingredient carts, cover exterior surfaces of freezer; cloth towels being used to line shelving; raw wood being used as shelving throughout facility, including in walk-in cooler and walk-in freezer.
      • Cardboard boxes being used to store cooked foods such as noodles and single service utensils; non-food grade containers being used to store foods in, in walk-in freezer and walk-in cooler; cloths being used to cover various foods in reach-in refrigerators; cutting boards severely scored and no longer easily cleanable.
      • Grocery bags were used rewrap foods in, in the Vissani chest freezer.
      • The wastewater pipes from the rinse and sanitize basins of the 3 compartment sink extend down into the floor drain, eliminating the air gaps; the wastewater pipe from the wash basin appears to circulate through 2 grease traps in series without exiting to a floor drain.
      • There is a huge amount of debris build-up on the floors, walls and equipment throughout the kitchen. Piles of old food/grease could be observed on parts of the floor, especially on, under and around equipment under the hood ventilation system.
      • Observed broken light fixture in hood ventilation system. (Note: hood ventilation system cleaned 7/6/2023)
      • Adequate lockers or other suitable facilities are not provided for employees clothing and other possessions resulting in personal items being stored throughout the kitchen near food and food contact equipment (single service items).
  • Bangkok Street, 3974 Fettler Park Dr Dumfries, VA 22025
    • Date inspected: July 19
    • Total violations: 2
      • Employees observed working in the food service area without proper hair restraints.
      • In-use utensils improperly stored between use.
  • La Flor de Puebla Bakery Cafe, 14335 Jefferson Davis Highway Woodbridge, VA 22191
    • Date inspected: July 19
    • Total violations: 16
      • Access to the handwashing along the cookline is restricted due to the placement of the 4 individual stockpot stoves and a shelving unit.
      • The chlorine sanitizing solution dispensed in the CMA Energy Saver stationary, low temperature warewashing machine was not at an acceptable concentration. A fresh bottle was added, but the concentration was still measured at 0ppm chlorine.
      • Spray bottles of chemicals in front service area unlabeled.
      • Medication (ibuprofen) was found stored on shelving adjacent to and/or above foods.
      • Spray bottles of chemicals stored near clean equipment (Hamilton Beach blender) in front service area.
      • Spray bottle of water, similar to spray bottles of chemicals, found in baking area unlabeled.
      • Foods being prepared at the prep table in the bakery area are subject to contamination from the closely adjacent hand sink.
      • Scoops used to dispense ice in service area and those in bulk containers were observed to be improperly stored in between usage with the handles down in the foods.
      • The chlorine sanitizing solution dispensed in the CMA Energy Saver stationary, low temperature warewashing machine was not at an acceptable concentration. A fresh bottle was added, but the concentration was still measured at 0ppm chlorine.
      • Wooden utensils that were stained and broken were being used for cooking.
      • Cardboard being used to line shelving and trays; raw wood shelving in bakery area; raw wood stool/transport.
      • Observed milk crates to be used as shelving and also for storage of single service items.
      • The 3 plugs/stoppers for both of the 3 compartment sinks are missing. Plastic bags are being used.
      • There is a buildup of debris on the ware-washing machine.
      • The shelving in the walk-in cooler has a large amount of food debris.
      • The PVC wastewater pipes of the 3 compartment sink in the kitchen area extend down into the floor drain, eliminating the air gaps.
  • Chunky Cup, 12723 Apollo Drive Woodbridge, VA 22192
    • Date inspected: July 24
    • Total violations: 13
      • The Person In Charge (PIC) was not able to provide documentation that all food employees or conditional employees were trained and informed of their responsibilities to report symptoms or diagnosis of, or exposure to, diseases transmissible through food.
      • This food establishment has not established written procedures for responding to vomiting or diarrheal events that happen in the facility.
      • Open drinking containers were stored (on the food prep table) in a manner that may contaminate food, food contact surfaces or utensils.
      • Observed an employee wash and rinse equipment in the 3 compartment sink and place it back into use, without sanitizing it. When asked, 2 employees on site did not understand the 3 step process. No chlorine on site (other than bleach product with fresh sent).
      • The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: chicken tenders @ 53°F and chicken wings @ 56°F in the Quiznos reach-in refrigerator.
      • Observed that there were (2) 12 quart containers of chicken wings on site that per the owner & CFPM were received raw, cooked, cooled, then stored in the 2 door upright freezer prior to storing in the 2 door upright refrigerator for re-heating and serving. There are inadequate facilities in the establishment for proper cooling down of this amount of cooked chicken wings.
      • The person in charge (PIC) could not provide a metal-stemmed probe food thermometer that was capable of measuring food temperatures.
      • Clean equipment, such as tongs and metal containers, was stored with the food contact surfaces up and exposed to dust and other contamination (hands).
      • Jelly topping containers being reused to store other foods in throughout the kitchen.
      • Single service spoons were found to be displayed unprotected (without proper dispensers) and exposed to contamination with the mouth contact surface exposed.
      • Cardboard being used to line shelving/surfaces.
      • The Quiznos 2 door reach-in refrigerator was not holding a temperature of 41 degrees F and below.
      • There is no test kit located in the facility for monitoring the concentration of the chemical sanitizing solution (chlorine).
