Woodbridge Health Inspections: Chick-Fil-A, Everbrook Academy

The Prince William Health District recorded violations at 3 facilities during a recent round of inspections in Woodbridge.

WOODBRIDGE, VA — Several restaurants around Woodbridge recorded violations during health inspections from Nov. 23 to Nov. 29, according to the Prince William Health District.

Most COVID-19 restrictions have relaxed around the state, and health inspectors are returning to conduct many restaurant and other health inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.

Violations typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection. Other violations are addressed later, and an inspector may conduct a follow-up inspection to ensure compliance.

  • Old Hickory Golf Club, 11921 Chanceford Dr Woodbridge, VA 22192
    • Date inspected: Nov. 28
    • Total violations: 24
      • This food establishment has not established written procedures for responding to vomiting or diarrheal events that happen in the facility.
      • The following handwashing sinks were found to have hot water at less than 100 degrees F: *handwashing sink in bar at 80 degrees F.
      • No hand towels available at hand sink in the bar.
      • The handwashing sink in the bar was being used as a dump sink with beverage debris found in it.
      • Raw eggs stored over large bulk container of ready to eat (RTE) (drink) lemonade.
      • Various foods in True upright freezer open and exposed.
      • Interior of ice machine unclean.
      • The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: slice cheese in the WIC at 43 degrees F and half and half in the WIC at 44 degrees F.
      • Spray bottles of toxics stored hanging near single service cups in bar.
      • The thermometer on the walk-in freezer was in disrepair.
      • The wet wiping cloths throughout the kitchen were not properly stored in the sanitizer bucket in between use.
      • Knives stored in space in between True 3 door prep refrigerator and prep table.
      • Single service container without a handle being continuously used to scoop bulk foods such as sugar and flour.
      • The ice maker was found to be in disrepair with the lid broken on one side.
      • Tape was used on the broken rice container.
      • The following refrigeration units were not holding temperatures of 41 degrees F and below: walk-in cooler (WIC) 42 degrees F; True upright refrigerator 42 degrees F
      • Bread crates were being used as shelving in the dry storage area.
      • There is a huge cascading "waterfall" of ice from the condenser in the walk-in freezer.
      • There is a buildup of debris on the warewashing machine.
      • An irreversible registering temperature indicator was not provided for the hot water mechanical warewashing operation.
      • The wash solution temperature for the Ecolab stationary rack warewashing machine was observed to be 144 degrees F. The rinse solution was observed to be 191 degrees F. The thermolabel did not turn black. Sanitizing of equipment is not occurring.
      • The following non-food contact surfaces were visible soiled: 1. The interior and exterior surfaces of equipment such as the CresCor hot holding cabinet, refrigeration units, etc
      • There is a "Y" adapter on the faucet at the mop sink that allows 2 hoses to be attached. One hose was observed to be fully extended into the basin. Mop sink faucet vacuum breakers are not designed for the continuous pressure created by the "Y" adapter/hoses.
      • The hood system, including the filters, over the cookline is soiled.
  • Chick-fil-A, 4101 Old Bridge Rd Woodbridge, VA 22192
    • Date inspected: Nov. 28
    • Total violations: 2
      • The concentration of the quaternary ammonium solution was measured using the test kit at the 3 compartment sink and was found to be above the desired concentration (toxic). This concentration exceeds the approved limits for use with food-contact surfaces of equipment and utensils in a foodservice establishment.
      • The urinal in the men's restroom is in disrepair and is covered and labeled to avoid use.
  • Everbrook Academy, 12725 Ridgefield Village Woodbridge, VA 22193
    • Date inspected: Nov. 28
    • Total violations: 5
      • There is no Certified Food Protection Manager (CFPM) on duty during hours of operation as those employees with supervisory and management responsibilities have no certifications. Note: Kitchen staff does have a ServSafe Food Handler Certificate.
      • The Person In Charge (PIC) was not able to provide documentation that all food employees or conditional employees were trained and informed of their responsibilities to report symptoms or diagnosis of, or exposure to, diseases transmissible through food.
      • This food establishment has not established written procedures for responding to vomiting or diarrheal events that happen in the facility.
      • Employees handling food were observed to not be wearing proper hair restraints.
      • Extensions have been placed on the original wastewater pipes of the 3 compartment sink so that they now extend down into the floor drain, eliminating the air gaps.
