Woodbridge Restaurant Inspections: 23 Violations At 1 Facility

The Prince William Health District inspected several facilities around Woodbridge and Dumfries during recent restaurant visits.

WOODBRIDGE, VA — Several facilities around Woodbridge recorded violations during the most recent round of health department inspections from May 1 to May 9.

Most COVID-19 restrictions have relaxed around the state, and health inspectors are returning to conduct many restaurant and other health inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.

Violations typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection. Other violations are addressed later, and an inspector may conduct a follow-up inspection to ensure compliance.

  • Jersey Mike's Subs, 3282 Old Bridge Rd Suite 107 Lake Ridge, VA 22192
    • Date inspected: May 1
    • Total violations: 23
      • The Person In Charge (PIC) was not able to provide documentation that all food employees or conditional employees were trained and informed of their responsibilities to report symptoms or diagnosis of, or exposure to, diseases transmissible through food.
      • Personal drink containers (gatorade) were stored on shelving with food (chips) in a manner that may contaminate food, food contact surfaces or utensils.
      • Food employees failed to wash their hands prior to donning clean disposable gloves for the handling of ready to eat (RTE) foods.
      • No hand towels available at hand sink in the back as the battery is out and hand towels can not be dispensed properly.
      • Pitcher found in handwashing sink near 3 compartment sink.
      • Deli meat slicer (back-up, not used during inspection), "clean" knife found on knife holder above 3 compartment sink, prep table in back and interior of Turbo Air 1 door reach-in refrigerator unclean with food debris accumulated on/in each of them. Staff indicated that slicers are only cleaned at the end of the day.
      • The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: ham and prosciutto in the Turbo Air reach-in refrigerator; ham and turkey in the display refrigerator; cut tomato and shredded lettuce in the front and back counter refrigeration units.
      • Deli meats and cheeses, out of original container, held for > 24 hours were not date marked.
      • The concentration of the quaternary ammonium solution, measured using the EHS's test kit in the sanitizer bucket, was found to be above the desired concentration @500+ppm (toxic). This concentration exceeds the approved limits for use with food-contact surfaces of equipment and utensils in a foodservice establishment.
      • Spray bottle of sanitizer and bucket of sanitizer stored over boxes of single use food contact gloves.
      • Observed large lexan container of tomatoes just sliced (several layers deep) at 66 degrees F. When employee was questioned as to cooling down practices, he responded that some of the product would be placed directly on the 2 (front and back) refrigeration inserts for immediate use and the rest would be placed in the covered lexan container in the walk-in cooler.
      • There was no ambient air temperature measuring device (thermometer) in the display refrigerator or in the Turbo Air 1 door reach-in refrigerator.
      • Various foods in white bulk storage containers were not labeled properly with the common name of the food.
      • Observed a fly in the kitchen near the cookline.
      • Employee (PIC) was observed to be wearing a severely soiled white polo shirt.
      • Employee (cook) handling food was observed to not be wearing a proper hair restraint.
      • Utensils used to dispense ingredients from bulk containers were observed to be improperly stored in between usage with the handles down in the foods.
      • There was a large build up of ice in the walk-in freezer noted during the 10/31/2022 inspection and a smaller amount noted during today's inspection; there was an ice build up beneath the back refrigeration insert.
      • The following equipment is in disrepair: 1. lexan containers broken.
      • The floors throughout the kitchen are unclean with visible debris and greasy build up.
      • Wet mop observed stored down in mop sink; other mop heads stored in milk crate on floor.
      • Observed inadequate lighting over cook area with 2/3 light bulbs burnt out.
      • Keys found on prep table; aprons stored hanging near drinks on back storage shelving; visor found stored on single service items on shelving near handwashing sink. Adequate lockers or other suitable facilities are not provided for employees clothing and other possessions resulting in personal items being stored on shelving used for food and single service items.
  • Taco Bell #39091, 13880 Shoppers Best Way Woodbridge, VA 22192
    • Date inspected: May 1
    • Total violations: 4
      • The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: guacamole w/ diced tomatoes, made onsite, stored on top of line 1 reach-through refrigerator.
      • The thermometer in the prep reach-through refrigerator #1/2 appears to be in disrepair measuring 46 degrees F while foods measure 41 degrees F and below.
      • There is a "Y" adapter on the faucet at the mop sink that allows 2 hoses to be attached. Mop sink faucet vacuum breakers are not designed for the continuous pressure created by the "Y" adapter/hoses.
      • An extension has been placed on the original wastewater pipe from the rinse compartment of the 3 compartment sink so that it now extends down into the floor drain, eliminating the air gap.
  • Fu Sheng, 18011 Dumfries Shopping Plz Dumfries, VA 22026
    • Date inspected: May 1
    • Total violations: 12
      • There is no person in charge with a valid Food Protection Manager Certification from an accredited agency recognized by the Conference for Food Protection.
      • Management, food employees or conditional employees were not aware or informed of their responsibilities in reporting symptoms or diagnosis of diseases transmissible through food.
      • This food establishment has not established procedures for cleaning up vomiting or diarrheal events that happen in the facility.
      • The handwashing facility located in the kitchen is blocked, preventing access by employees for easy handwashing.
      • Observed that hand soap was not provided at each handwashing sink.
      • No hand drying provisions were provided at the handwashing sink in the in kitchen.
      • Raw animal foods are not separated from each other during storage, preparation, holding, and/or display.
      • Equipment food contact surfaces/utensils were not observed cleaned between working with raw foods and ready-to-eat foods.
      • Unlabeled food containers with the following food items that are not easily identified by appearance.
      • Food stored in a location where it is subject to splash, dust or other contamination.
      • Employees observed working in the food service area without proper hair restraints.
      • The cutting board(s) along the prep cooler are heavily scratched and scored. The food contact surface is no longer easily cleaned and sanitized due to condition.

    • Total violations: 1
      • The covering over the wall is chipped and no longer easily cleanable.
