Woodbridge Restaurant Inspections: Dunkin' Donuts, Baskin Robbins
Several facilities around the Woodbridge area recorded violations during recent inspections from the Prince William health department.
WOODBRIDGE, VA — Several restaurants around Woodbridge recorded violations during health inspections from Nov. 16 to Nov. 22, according to the Prince William Health District.
Most COVID-19 restrictions have relaxed around the state, and health inspectors are returning to conduct many restaurant and other health inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.
Violations typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection. Other violations are addressed later, and an inspector may conduct a follow-up inspection to ensure compliance.

- An extension has been placed on the floor drain so that the waste water pipes from the 3 compartment sink now extend down into the floor drain, eliminating the air gaps.
- There is a "Y" adapter on the faucet at the mop sink that allows 2 hoses to be attached. Mop sink faucet vacuum breakers are not designed for the continuous pressure created by the "Y" adapter/hoses.
- The walk-in cooler (WIC) was not holding a temperature of 41 degrees F and below at the time of the inspection. - This facility has a 2 compartment sink used to wash and rinse equipment with a Hobart warewashing machine which is used for the final sanitizing step. The Hobart warewashing machine reached a temperature of 90 degrees F during the inspection. In addition, the irreversible registering temperature indicator (thermolabel) failed to change from white to black. The warewashing machine is not sanitizing the equipment at a surface temperature of 160 degrees F as required.
- The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: cheese @ 43 degrees F and butter @ 43 degrees F in the True refrigerator.
- The following equipment is in disrepair: 1. serving bowls are discolored and scored 2. the hood system and filters are discolored and rusted 3. the blade of the can opener attached to the prep table is discolored and rusted
- The floors around the equipment and under the shelving in the kitchen area were unclean with visible soil. Per staff, it is a build-up of wax that results in the black debris on the floors.
- Adequate lockers or other suitable facilities are not provided for employees clothing and other possessions resulting in personal items being stored on shelving used for storage of single service items and foods.

- This food establishment does not have written procedures for cleaning up vomiting or diarrheal events that happen in the facility.

- Jersey Mike's Subs, 3282 Old Bridge Rd Suite 107 Lake Ridge, VA 22192
- Date inspected: Nov. 16
- Total violations: 3
- Cut tomatoes (covered) in the walk-in cooler @ 48 degrees F; cut tomatoes on front prep unit @ 41 degrees F; cut tomatoes on back prep unit @ 54 degrees F; shredded lettuce on front prep unit @ 54 degrees F and shredded lettuce on back prep unit @ 56 degrees F. Per Jarrod, tomatoes are purchased unripe and allowed to ripen at room temperature. Staff sort through tomatoes daily and refrigerate (in the walk-in cooler) those that are to be sliced the next morning. Tomatoes are sliced and then either placed in the walk-in cooler or on one of the prep units (front or back). Cooling the tomatoes overnight in the walk-in cooler before slicing is good, but the cut tomatoes should also be cooled to 41 degrees F or below, uncovered, up high, in the walk-in cooler prior to placing on the prep units (front or back) or storing covered in walk-in cooler. This same procedure should also be used for the shredded lettuce. Cut tomatoes and shredded lettuce out of temperature today, >41 degrees F (cut/shredded at 7:30am) were discarded today as it had been 4+ hours.
- Ham @47 degrees F and cheese @ 46 degrees F in the display refrigerator were also still above the required 41 degrees F or below. Per Jarrod, Jersey Mike's is exploring alternative refrigeration units that can handle being opened frequently. Unit set at 33 degrees F to facilitate holding foods at 41 degrees F or below at all times.
- During today's inspection, a large build up of ice was noted in the back left corner of the walk-in freezer. Adjust/repair unit so that there is not thawing and refreezing going on in the unit.

- This food establishment has not established written procedures for responding to vomiting or diarrheal events that happen in the facility.
- The handwashing sink in the ice cream area was being used to rinse equipment such as the blender containers.
- There was no soap available at the handwashing sink in the ice cream area. The dispenser has fallen off of the wall.
- Chemicals stored near clean equipment in warewashing machine area.
- There were no ambient air temperature measuring devices (thermometers) in the Silver King 1 door reach-in refrigerator or the Delfield 1 door reach-in refrigerator (near donuts). The thermometer in Delfield 4 drawer reach-in refrigerator was in disrepair.
- Food stored on the floor in the WIF (ice creams) and the dry storage area (boxes of juices).
- Employees handling food (donuts) were observed to not be wearing proper hair restraints.
- Wet wiping cloths (blue) throughout the establishment were not properly stored in sanitizer buckets in between use.
- Scoops for ice cream were observed to be improperly stored in between usage in standing water at room temperature in 1 dipper well (left); other equipment (knife, etc) was stored in a red cup labeled sanitizer at the handwashing sink up front; ice scoops in the drive-though area and in the ice machine in back were stored with the handles down in the ice.
- Boxes of cups in the back area and boxes of combo bags in the dry storage area were stored on the floor.
- The following non-food contact surfaces were visible soiled: 1. The shelving in the walk-in cooler (WIC)

- There is no test kit located in the facility for monitoring the concentration of the chemical sanitizing solutions.

- There was no soap available at the small handwashing sink along the service line in the Bistro.
- No hand towels or dispenser available at hand sink along the service line in the Bistro.
- Raw beef and salmon stored over ready to eat (RTE) diced cooked chicken in the True 2 door refrigerator near the office.
- Beef Tenderloin, per staff, is the only food item that can be ordered undercooked, to preference. This is only offered to the Independent Living (IL) residents NOT the Assisted Living (AL) residents. While the Consumer Advisory is on the menu for the main dining hall, there is no marking to indicate which food it is referring to on the menu.
- The Alto Sham in the Bistro is in disrepair.
- The following refrigeration units were not holding temperatures of 41 degrees F and below: True refrigerator in tray line area @ 45 degrees F; True 2 door refrigerator near office @ 42 degrees F; WIC #1 (produce) @ 42 degrees F AL True 1 door upright refrigerator @ 52 degrees F In general, these units also had foods at slightly higher than the required 41 degrees F and below for cold holding of time/temperature control for safety (TCS) foods. *see temperature charts
- Milk crates and bread trays being used as shelving in both the walk-in freezer and the dairy walk-in cooler.
- The wastewater lines from the 3 compartment sink in the Assisted Living (AL) kitchen are directly plumbed into the sewage system without air gaps.

- Date inspected: Nov. 21
- Total violations: 2
- Temperatures of tomatoes and pre-cooked wings in the 2 door prep refrigerators along the cookline are still slightly elevated at 42 degrees F. Other items such as lettuce, shredded cheese and pre-cooked pasta are below 41 degrees F as required. Discussed the proper cooling methods for both the tomatoes and pre-cooked wings with the CFPM.
- A new "Y" connection has been placed on the mop sink. Ensure that there is a back flow prevention device in the "Y" adaptor or add the double check valve as per the picture provided.
