Sign up for Affordable Health Insurance at the South San Francisco Community Health Fair.

WHEN: Saturday, Jan. 10, 2015, 9:00am to 12:00pm
WHERE: South SF Municipal Services Building, 33 Arroyo Drive, South San Francisco

Please bring your ID, proof of citizenship or legal residency, and proof of income for you and your household. You may qualify for financial assistance offered by Covered California or Medi-Cal. You can apply even if not all of your family members are eligible.

We will have bilingual staff available to answer all your questions about health insurance. We can help you and your family enroll in an affordable health insurance plan under Covered California and the Affordable Care Act.

A partnership between the City of South San Francisco, Office of Supervisor Dave Pine, San Mateo County Health System, San Mateo County Human Services Agency, and Covered California, the community health fair aims to connect San Mateo County residents with affordable healthcare insurance before the final enrollment date on February 15.
