Person in Charge removed lids from containers after taking temperature. Also added ice to sauce. Ensure proper and effective cooling methods are used. Observations: Employee handling baked biscuits with bare hands. Ice machine contained mold-like substance buildup inside machine. Cold-held potentially hazardous foods not maintained below 41F; no time controls/documentation in place. After discussion with CFSM and other manager, sliced tomatoes and sliced/shredded cheese held under Time as a Public Health Control and marked for four (4) hour hold time. Sliced cheese and shredded cheese were not marked according to written procedures with sticker that contains start and discard time. Cut lemons placed at drink station for customer self-service in container that requires customers to remove lid and provides no further shielding/protection from contamination (ex. sneeze guard). Garbage receptacle sliding doors left open and not in use. Lids and doors shall be kept closed when not in use.

Observations: The Person in Charge (PIC) unable to convey knowledge of the employee health policy; unable to state the 5 diseases, all of the 5 symptoms, and Regulatory Authority reporting requirements. Employee drinks (in bottle, in cups with no lids/straws) found in the kitchen. Some drinks were stored next to food and pans. Cloth towel, sponges stored on hand sink. Items removed at time of inspection.A handwashing sink shall be maintained so that it is accessible at all times for employee use. Multiple dented cans found on can rack. Mold-like buildup inside the ice machine. Pizza sauce not cooling at an adequate cooling rate. Soap solution and sanitizer solution, which are used to clean the dining tables, were not labeled. Sanitizer buckets stored adjacent to food, pizza pan rack, on the front of house storage shelves. Pasta, pizza sauce cooling in deep, covered, plastic food containers, below food held cold, on the bottom shelf of the walkin cooler. Scoop handles were stored in direct contact with food in the cold top cooler. Both quat test kits were damaged; quat sanitizer solution is used in the sanitizer buckets & in the dish sink. Sticker residue left on clean food containers. Dust buildup on th walkin cooler fan guards. Mold-like buildup on the nonfood contact surface of the self-service drink dispenser ice chute. Carbon-buildup on the pizza pans. Water leaking from the prep sink drain line, onto the floor below. Follow-up within 10 days.

Observations: Raw steak stored above boxes of raw bacon in walk-in cooler. Tray of cooked chicken stored on top of container of raw pork ribs and directly under container of raw fish. Employee handled raw beef patties and proceeded to handle bread for service. Container used for carrying raw chicken from prep station to fryer also used for carrying raw shrimp. Upon beginning inspection, employee in back of kitchen handling and preping raw chicken wearing a watch. Employees with hair/beard longer than 1/2 inch have no proper hair restraint for hair or beard. Knifves stored between uses, stored between lid and side of cooler that contains food residue buildup. Single-use gloves used for multiple tasks that present risk for cross-contamination. Employee talking and handling cell phone while preparing food.
