Observations: Employee not washing hands while changing gloves to handle food on multiple occasions. Employee handled raw sausage patties, proceeded to handle food press with gloved hands. Employee then changed gloves handled press again and then grabbed raw bacon. Employee cracked raw eggs and then proceeded to grab sliced bread. Stopped employee before grabbing bread and had them change gloves. Tables being cleaned with single sanitizer towel. Had tables cleaned with two towels. Hydrated hash browns (2 containers) (hydrated 6/2) temped at 56F. Food not cooled below 41F within four (4) hours. Dishware stored at bar seating area not inverted or covered. Single-use gloves not being discarded after handling raw animal foods and before handling plates or other ready-to-eat foods.

No paper towels at hand sink at wait station across from bar and paper towel dispenser not working at hand sink in kitchen. Cold-held potentially hazardous foods not maintained below 41F; no time controls/documentation in place. Multiple chemical spray bottles unlabeled with chemical in bottle. Flies observed throughout facility.

Ice machine with black mold like substance present around the food contact surface area. Milk, raw bacon & liquid eggs cold holding above 41 F (no time control documentation). Sausage gravy and sausage patties cooling for greater than 6 hours with temperatures above 41 F. Items were discarded. Sliced tomatoes, shredded cheese & sliced cheese on time controls (TPHC) not labeled with the date, start time and/or discard time. Current inspection report not posted.
