Observations: No active managerial control. Violations for hand washing, knowledge of the employee health policy, using a hand sink, cleaning & sanitizing food contact surfaces, reheating for hot holding, cold holding (amongst other good retail practices) were observed today. Food employees not cleaning hands immediately before engaging in food prep; between glove changes; after handling money. Employees were not using proper cleaning procedure: washing <20 seconds, using cold water, turning the water off with clean hands, etc. Training provided; employees washed their hands again. PIC unable to convey knowledge of the employee health policy: symptoms, illnesses, exclusion/restriction requirements, reporting requirements. A disposable wiping cloth was stored in the back hand sink. Food debris left on the slicer that was stored clean on the prep table shelf. Observed PIC cleaning the dining tables with a disposable cloth & a spray bottle. Potentially hazardous foods reheated for hot-holding not reheated so that all parts of the food reach a temperature of at least 165F for 15 seconds. Cold-held potentially hazardous foods not maintained below 41F. No small-diameter probe thermometer available for measuring temperatures of thin-massed foods. Women’s restroom door is not completely closing; not tight-fitting.

Observations: Employee drinking from an unapproved container (glass bottle) while cooking food on the stove. Employee drink cup stored on the prep table by the stove, next to in-use utensils. Employee drink in an unapproved container (water bottle) stored on the table next to single-use items. Unapproved drink containers were discarded. The approved drink container was moved to a storage shelf. Cold-held potentially hazardous foods not maintained below 41F; no time controls/documentation in place. *One container of pimento cheese not cooled to 41F within 4hours. Food measured 44.5F in the center, 42F on the edge. Food was cooling in a deep, covered, plastic container since yesterday. Food discarded; training provided on cooling & cooling methods. Refrigerated, ready-to-eat, potentially hazardous food prepared and held in the establishment for more than 24 hours not clearly marked to indicate the date by which the food must be consumed, sold, or discarded: salsa, cheese, pico, American cheese, succotash (cooked vegetables), cooked beef. Food stored on the floor in the front room. Permit not displayed. PIC unable to find permit. The current inspection report was not posted. The “No Smoking” sign was not posted in a conspicuous place. ap at the bottom of the back exit door; daylight visible in the bottom corner. Back exit door doesn’t self-close all the way; not tight-fitting. Multiple flies found in the facility.

Observations: No active managerial control. Violations for food in good condition safe and unadulterated, food separated and protected, food contact surfaces cleaned and sanitized, and date marking (amongst other good retail practices) were observed today. Foods not safe, adulterated, and/or not honestly presented. Bowl used to portion raw beef was stored directly on top of carrots in the cold-top cooler; food adulterated. Food discarded. Raw beef stored above raw pork in the freezer. Raw chicken and raw pork potstickers stored above crab sticks in the freezer. Unwashed produce stored above ready-to-eat food (sauces) in the walkin cooler. Food separated & organized. Food storage chart was posted on the wall. Mold-like buildup inside the ice machine. Cooked foods and ready-to-eat foods (spring rolls, bbq pork, crab rangoon, cooked meats, etc), that are cooked onsite and then stored in the freezers, are not date marked. Foods made with imitation crab are listed as crab or crab meat on the menus. Seasonings, msg, cornstarch, sauces, etc at cookline were not labeled with the common name. Foods were cooling using ineffective cooling methods. Cooked noodles, hydrating rice noodles were cooling in deep, covered, plastic containers, in the walkin cooler, below food held cold. Raw chicken cooling in deep, covered, plastic containers, stacked 2 high, in the walkin cooler. Chicken was being thawed in the dish sink, while the dish sink had dirty dishes stored on the drainboard & in the adjacent compartment - contamination risk. Food discarded. Wet wiping cloth stored on top of reachin cooler. Rusted shelves in the reachin cooler. Grease/food buildup on the cookline equipment. Men’s and women’s restroom doors don’t close completely; not tight-fitting. Coved base and floor tiles under the cookline equipment are damaged & lifting; not smooth & easily cleanable. Employee food stored above customer food in the freezers. Solid back door left open; screen door left open.
