Observations: Person in charge not performing responsibilities. Observed hot holding, cold holding, bare hand contact and sanitization violations. Person in charge is responsible for food safety. Observed employee cutting and handling a lemon with bare hands. Dish machine not reaching a temperature of 160 to sanitize dishes and reading 0 ppm chlorine. Dish machine is set at to high a temperature for chemicals, but the booster heater only reaches 156 when dishes are being washed. Dishes manually sanitized. Steak, pasta salad, and mozzerella cheese in salad cooler not maintained at 41F. Meatballs at back steam table below 135F. Several items on self-serve bar not maintained at 135F. All food reheated. Spray bottle and chemical bottle not labeled with common name. Two cartons of milk were expired. Facility had bimetallic thermometer and no thermometer to measure thin masses of food. Unused dishes stored underneath 3 compartment sink on the floor. Dishes need to be removed if not using.

Observations: Non-food contact surfaces not clean. Exterior top portion of mechanical dish washer not clean. Must ensure all non-food contact surfaces are properly cleaned. Light fixtures located on lights in front kitchen prep area do not fit properly - end of light bulb is not shielded. Must ensure all light bulbs are properly shielded.
