Remarks: Observed interior of ice machine with mold-like build up. Observed butter blend, tomato sauce and turkey and cheese sandwiches that were above 41 F. Observed two cases of sterno fuel stored above food prep area and four cans of sterno fuel above single use items and food prep area. Observed several employees wearing jewelry including bracelets and rings while preparing food items. Observed four sanitizer buckets being stored on the floor. Observed unwashed avocadoes with sticker on being used in food prep area.

Disclosure statement and Reminder statement not in capital letters. The asterisk on the items offered undercooked not the same as the asterisk on the disclosure and reminder statement. Address immediately. Cold-held potentially hazardous foods not maintained below 41F; no time controls/documentation in place. Time on chicken on time as a control listed as 10:30-3. Chicken DISCARDED. Chlorine sanitizer >200PPM. Sanitizer diluted to 100ppm chlorine. Bottle of RAID for ants and roaches in facility. Had facility discard pesticide. Only pesticides approved for food service establishments may be stored and used in facility. Follow up within 10 calendar days.
