Foods (sliced tomatoes, salsa, rice, beans, cooked peppers, tamales) prepared Friday-Monday morning without dates of preparation in the coolers. Accumulation of debris on cooler handle, shelving, gaskets, and fans. Live roaches (3) in the prep area and dining room.

Observations: Employee restrained hair then wiped her face with her hand then put on gloves and began handling cheese. Person in charge could not answer questions about the health policy including what to report and to whom. Employee water bottle on counter in kitchen employee drink on prep area in bar area. Employee drinks moved. Items on breakfast bar under time as a control did not have a start or end time. Employee with long hair preparing food in the kitchen without any type of hair restraint. Wet wiping clothes stored on the counter tops.
