Observations: Foods stored uncovered in the walk-in cooler and freezer. Foods should be stored covered when not in use. Foods (sliced tomatoes, salsa, rice, beans, cooked peppers, tamales) prepared Friday-Monday morning without dates of preparation in the coolers. Accumulation of debris on cooler handle, shelving, gaskets, and fans. Live roaches (3) in the prep area and dining room.

Observations: Employee restrained hair then wiped her face with her hand then put on gloves and began handling cheese. Person in charge could not answer questions about the health policy including what to report and to whom. Employee water bottle on counter in kitchen employee drink on prep area in bar area. Employee drinks moved. Items on breakfast bar under time as a control did not have a start or end time. Employee with long hair preparing food in the kitchen without any type of hair restraint. Wet wiping clothes stored on the counter tops.

Observations: Multiple TCS foods (time/temperature controlled for safety foods) were not reheated to at least 165 F for 15 seconds within two hours: mushroom cream sauce, collard greens, and demi mushroom sauce. Inspection report is not prominently displayed. Report is hidden behind a raised portion of the bar countertop. Lower the bar top or move the report to a different location.

Observations: Food residue and debris remaining on containers, plates, bowls, etc while they items were air drying. Several TCS food (time/temperature controlled for safety) items were prepared then held for more than 24 hours without dates. Shelves in the walk-in cooler have accumulation of mildew. Sticker residue left on multiple containers.
