Physical Therapist for Multidisciplinary Clinic

We are an established wellness center with a substantial patient base and we are looking to expand. We are seeking a dynamic and motivated PT for a Team Lead position at our South Elgin location. This is an exciting opportunity for a PT that is looking to branch out. Candidates must be licensed and have the desire to excel in the field of Physical Therapy. Physical Therapist Team Lead responsibilities:

• Perform evaluations and re-evaluations of new and existing patients.
• Provide patients with physical therapy services, according to facility regulations.
• Ensure that all short term and long term goals are attained.
• Be responsible for monitoring and documenting patient responses to all clinical interventions and findings.
• Play an active role in the discharge planning process by working directly with family members and physicians when necessary.
• Be flexible and willing to perform any other duties and tasks assigned to the individual therapist.

Requirements:

• You must be a Physical Therapist or Physical Therapist Assistant currently licensed in the State of Illinois

• Experience a plus but not necessary/New graduates welcome to apply

• Spanish speaking is a plus but is not necessary
