A Year of Progress at Bay Harbor Post Acute and Healthcare in Salisbury, Md.

New leadership, programming enhancements and a multi-million-dollar renovation punctuate a year of significant progress following a rebranding at Bay Harbor Post Acute and Healthcare Center in Salisbury. Located at 200 Civic Avenue, the 305-bed skilled nursing center provides post-hospital care, short-term rehab and long-term residential care.

“What a tremendous difference a year makes!” said Jennifer Carnegie, regional director of market development at Marquis Health Consulting Services, which supports Bay Harbor. “This center’s top-to-bottom modernization responds directly to demand for modern, quality skilled nursing options within the Wicomico County region.”

Over the past year, the center, formerly Salisbury Rehabilitation and Healthcare Center, has enhanced its leadership to complement the talented in-place team. The new appointments include Medical Director Sufyan Malik, MD, and Subacute Medical Director Shahabuddin Soherwardi, MD. Both well-respected physicians are affiliated with TidalHealth.

Additionally, Bay Harbor is undergoing a full repositioning. This includes renovations – which are touching every part of the building – and additions to the facility’s specialty care offerings, which are designed to meet area healthcare needs.

To that end, Bay Harbor has an in-facility, 24-chair hemodialysis den, which is under the direction of nephrologists Kazi Khan, MD, and Eric Ofori, MD. The den operates in partnership with Concerto Renal Services.

In addition, Bay Harbor:

• Recently introduced OrthoWIN, a highly specialized subacute care program for orthopedic recovery, led by Bethany Klopfer, APRN FNP-C, a Physical Medicine and Rehabilitation (PM&R) specialist.

• Is pursuing an advanced certification for its established Stroke Recovery program.
