Report Confirms Operator at Fault for Braintree Runaway Train
The MBTA released the report Tuesday.

BRAINTREE, MA - A report from the MBTA confirmed that the operator of a Red Line train that left Braintree unmanned was at fault for the December 2015 incident.
WCVB reports that former MBTA operator David Vazquez tied a cord around the "dead man's switch" before accessing an emergency bypass switch. As he left the train, he forgot to remove the cord and set the hand brake. When Vazquez activated the emergency bypass switch, the train left the station because the controller was set to full power.
The train went through Quincy before MBTA workers turned off the third rail, bringing the train to a halt just past North Quincy.
