The Concord SEPAC and the Concord Public/Concord-Carlisle Regional School District are pleased to offer a Transition Overview Workshop designed to help you prepare and plan for your child’s postsecondary needs and goals. This is the first of a three part series on transition.

The Transition Overview will include information on:

  • Transition assessment, planning and services in 
Massachusetts, including the Transition Planning Form. 

  • Review of transition laws (State and Federal).
  • When to start developing independent living skills.
  • Post-secondary agencies and community resources.
