Clark Health Department To Offer Flu Shots At Recreation Center
The health department and CVS will offer the shots Monday, Oct. 5 in the center's cafeteria.

The Clark Health Department in conjunction CVS Pharmacy, 60 Westfield Ave., Clark will be offering flu shots to adults ages 18 and up on Monday, October 5, 2020 between 10:00 a.m.-1:00 p.m. at the Clark Recreation cafeteria. Parking is available by the tennis courts.

Interested participants will be asked to complete a health and wellness check list prior to administration of the flu shot. All participants must bring all insurance cards (Medicare, HMO’s etc.) so that CVS can bill for reimbursement of the shot. Cash and check transactions cannot be accepted.
You may request the forms in advance by calling Nancy Raymond, Health Officer at 732-428-8405. Please leave your first and last name, address and how many forms you are requesting. On the day of the vaccination, there will only be one way into the building by the gymnasium doors and one way out through the cafeteria.

Entrance and exist will be marked. Social distancing of 6 feet will be required. Face coverings must be worn and must cover your nose and mouth at all times.
You do not have to be a resident of Clark to receive the flu shot. Walk-ins are welcome.
