High NCQA Ranking for Southampton Health Center

The HRHCare Kraus Family Health Center at Southampton is named a Level 3 Patient-Centered Medical Home by the NCQA.

HRHCare, a nonprofit Federally Qualified Health Center offering a range of medical services for underserved communities throughout the Hudson Valley and Long Island, is proud to announce that the HRHCare Kraus Family Health Center at Southampton has been named a Level 3 Patient-Centered Medical Home (PCMH) by the National Committee for Quality Assurance (NCQA). The highest PCMH ranking available, this recognition solidifies the Health Center at Southampton, along with the 21 other HRHCare Health Centers that have already achieved Level 3 PCMH status, as a medical home that emphasizes care coordination and communication to transform primary care into what patients need and want it to be.

Saving the nation $24 billion annually and bringing $26.5 billion in economic activity to the communities they serve, 59% of all Community Health Centers have been recognized by the NCQA as Patient-Centered Medical Homes. This is directly tied to the Community Health Center model of affordable, comprehensive care; integrating medical, dental and behavioral health care along with other services to facilitate health care access and better health outcomes. Achieving Level 3 PCMH status depends on compliance with all of the PCMS must-pass elementsincluding data-proven standards for patient access and communication, implementation of evidence-based guidelines for chronic and other important conditions, active support of patient self-management and systematic tracking of test results and referrals.
