SV Spring Sports Physicals and Drug Testing

Spring sports physicals and drug testing will be offered to students in grades 7-12, at 2:30 p.m. in the senior high school.

For the physical, athletes are to submit a PIAA Comprehensive Physical Form. Sections 1, 2 and 3 must be completed by a parent or guardian and the doctor will sign section 4. The $15 fee may be paid in cash or a check payable to S.V.S.D. Forms are available on the internet at www.svsd.net under the athletics tab or in the athletic office.

For drug testing, students must bring a completed Seneca Valley Parental/Guardian/Custodian Drug Testing Consent Form which is available in the office or at www.svsd.net under the Athletics tab. The $28 fee may be paid in cash or a check payable to SVSD.
