Arlington County Public Health Department inspectors recorded violations at several food establishments during recent inspections.
ARLINGTON, VA — A number of facilities in Arlington recorded violations during recent inspections from the Arlington County Public Health Department. Inspectors conducted the most recent visits between April 9-16.
At one Arlington food establishment, an inspector reported seeing a container of calamari thawing in container on the floor under a hand sink.
"Protect food from contamination by storing the food in a clean and dry location," the inspector wrote as a corrective action. "Foods moved to 3 compartment sink area to finish thawing and preparation."

Store drinks away from prep areas. Beverages moved, lids provided for uncovered beverages.
- No parasite destruction records maintained for smoked salmon served in ready-to-eat form. Corrective Actions: Provide written agreements or statements from the supplier or aquaculturist that fish were raised and fed formulated feed, such as pellets, that contain no live parasites infective to aquacultured fish.
- Observed hash brown patties were holding at 50F in prep table cooler. Corrective Actions: Cold hold potentially hazardous food at 41°F or below to inhibit the growth of harmful bacteria. Hash browns were moved to interior of prep table cooler to cool.
- The person in charge was unable to provide written procedures for the use of time as a public health control for parcooked fries. Corrective Actions: Provide written procedures for the use of time as a public health control. The procedures must include the following steps or practices: a. Food must be marked with the time within which it must be cooked, served or discarded. b. The food is served or discarded within four hours from the time when the food is removed from temperature control c. Food in unmarked containers or packages, or for which the time expires is discarded and d. Written procedures are maintained in the food establishment and made available to the regulatory authority upon request e. Once time is implemented as a control measure for potentially hazardous food, no other measures may be substituted. During the inspection EHS provided TPHC policy, and PIC completed policy during the inspection. Ensure that the time policy is maintained on site and that fries are labeled.
- Smoked salmon and sunnyside egg offered on the breakfast menu, menu board, table tent or brochure do not provide a consumer advisory statement or disclosure statement. Corrective Actions: The Consumer Advisory shall include a disclosure statement that includes a description of the animal-derived foods offered raw or undercooked and identification of the animal-derived foods by asterisking (*) them to a footnote that states that the items are served raw or undercooked, or contain raw or undercooked ingredients.
- Improper methods used to thaw frozen meats. Observed meats thawing in food prep sink in standing water. Water was about 81F. and frozen sausage was about 48F. Corrective Actions: Thaw potentially hazardous foods by either of the following methods: 1. Under refrigeration that maintains food temperature at 41°F 2. Completely submerged under running water at a temperature of 70°F or below with sufficient water velocity to agitate and float off loose particles in an overflow 3. As part of the cooking process. Instructed staff that items had completed thawing and to move foods to cooler.
- Improper use of wet wiping cloths. Observed wet cloths used to stabilize cutting boards. Corrective Actions: Use clean wet wiping cloths for wiping spills from food-contact and nonfood-contact surfaces of equipment. Store wet wiping in a chemical sanitizer at the proper concentration between use.
- Establishment does not have QT-40 test strips for sanitizer solution. Corrective Actions: Obtain a QT-40 chemical test kit to ensure the sanitizing solution is maintained at the proper solution.

- Observed cartons of raw-shelled eggs stored over boxed lunches and salads in "Kratos" 2-dr reach-in ref. Corrective Actions: Separate raw animal foods during storage, preparation, holding, and display from raw RTE food including other raw animal food such as fish for sushi or molluscan shellfish, or other raw RTE food such as vegetables, and cooked RTE food.
- Observed equipment and utensils are not properly sanitized. When tested, quat ammonium chloride sanitizer was not at proper concentration; 3-compartment sink was not setup properly. Corrective Actions: Provide quat-ammonium chloride sanitizer at proper concentration of 200-400 ppm and immerse or expose food contact surfaces to sanitizing solution for adequate time.
- Observed interior of microwave oven not clean. Corrective Actions: Clean the microwave oven at least every 24 hours or as per manufacturer's instructions.
- Observed ham (48F), sliced provolone (55F), sliced roasted turkey (55) and grilled chicken (53F) at improper cold holding temperature in the "Hoshizaki" 2-dr sandwich prep ref. Corrective Actions: Relocate food to a refrigeration unit that is capable of maintaining food storage at 41°F or below.
- Observed some of the cutting boards heavily scored and scratched. Corrective Actions: Resurface or discard and replace the cutting boards to provide a food contact surface that is smooth, easily cleaned and sanitized.

- Observed that greens, cheese sauce were prepared the previous day, and had internal temperature of 67F, and 53F. The walk in cooler the items were placed in had an ambient temperature of 53F. Corrective Actions: Cool time/temperature control for safety (TCS) food(s) from 135°Fto 41°F within a total of 6 hours and from 135°F to 70°F within 2 hours. A longer time for cooling allows an ideal situation for bacterial growth. This has been shown to be the major contributing factor in many foodborne illnesses. Foods discarded.
- Observed raw eggs not stored in refrigeration equipment that maintains ambient air temperature of 45°F or less. Eggs stored in walk in cooler at 53F. Corrective Actions: Store raw eggs in refrigerated equipment capable of maintaining ambient air temperatures of 45°F or less. Discard the food and ensure that eggs are stored in equipment that can maintain temperatures of 45°F or below to inhibit the growth of harmful bacteria.
- The establishment does not have an approved HACCP plan for the food that is packaged using the reduced oxygen packaging (ROP) process. During inspection observed that establishment is vacuum packaging several items including raw hamburger meats, cooked sausage, cooked pulled pork, and cooked chicken and turkey
- Corrective Actions: Cease the service or sale of ROP food. An approved HACCP plan must be in place prior to packaging food using the ROP processing method. Develop a HACCP plan that: Identifies the time/temperature control for safety (TCS) food to be packaged, specifies that the food meets the required criteria, describes the label on the ROP food, includes required operational procedures, describes the training program for employees responsible for ROP operation, and is provided to AND APPROVED BY the Health Department before the ROP process is implemented. - In-use utensils found in standing water room temperature. - Shared strip mall trash & recycling containers aren't kept closed; carboard dumpster drain plug is missing. Corrective Actions: Cover all waste containers when not in continuous use. Replace the missing drain plug.
- Women's sink vanity cabinet door's are damaged. Wall panel not sealed flush with it's adjoining wall behind/above dish machine upper drying rack. Openings in ceiling around pipes shall be closed. Corrective Actions: Hand drying devices such as individual disposable paper towels, a continuous towel system that supplies the user with a clean towel or heated air hand drying device must be provided at all hand washing lavatories to encourage proper hand washing and avoid employees to drying their hands on their clothing or other unclean materials
Facility observed missing test strips for 3-compartment sink. Corrective Actions: Obtain a quaternary ammonium chemical test kit to ensure the sanitizing solution is maintained at the proper solution.

- Observed rice pudding, cheesecake and foods in the dessert prep table cooler were holding above 41F. Corrective Actions: Cold hold potentially hazardous food at 41°F or below to inhibit the growth of harmful bacteria. Relocate food to a refrigeration unit that is capable of maintaining food storage at 41°F or below.
- The methods used for cooling were not adequate. Observed that potatoes were cooled in covered containers and stacked. Corrective Actions: Arrange the food containers in the refrigeration equipment to provide maximum heat transfer through the container walls. Loosely cover or uncover the food if protected from overhead contamination during the cooling period to facilitate heat transfer from the surface of the food. Containers opened to allow cooling.
- Observed employee rinsing peppers in mop sink. Corrective Actions: Mop sinks may not be used for food preparation. Rewash peppers at a food prep sink or a cleaned basin on the 3 compartment sink only. Wash any containers and discard food in direct contact with mop sink.
- Observed container of calamari thawing in a container on the floor under the handsink. Corrective Actions: Protect food from contamination by storing the food in a clean and dry location. Foods moved to 3 compartment sink area to finish thawing and preparation.
- Observed that several cutting boards at prep tables are heavily scored. Corrective Actions: Sand down or replace heavily scored cutting boards. Boards with deep cuts are difficult to sanitize.
- Observed that lid of flour bin is cracked, and observed a food storage container was heavily cracked. Dessert prep table cooler is unable to hold foods at 41F. Corrective Actions: Discard equipment that is cracked or in disrepair. Container discarded, chef ordered new lid, and contacted servicer to adjust or repair prep table.
- Observed that the fans in the upright cooler are in need of cleaning. Corrective Actions: Clean the surface at a frequency necessary to prevent accumulation of soil residues.

What Violations Do ACPHD Inspectors Report?
In Virginia, all food service locations are supposed to be inspected at least once per year. Facilities with higher risk factors may be inspected as often as four times per year, according to the Virginia Department of Health.
Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected. Sometimes, the follow-up inspection only considers the most important "priority" violations.
For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection. Depending on the results of the inspection, the health department can pursue several actions.
They can issue a permit for the restaurant to continue serving food, they can schedule a follow-up inspection to determine if violations have been rectified, or they can temporarily shut down an establishment until certain issues are solved.
The Virginia Department of Health only recommends restaurants have their permits suspended when "the continued operation of the food service presents a substantial and imminent threat to the public health."
