Health Inspections: Dented Tomato Juice Can labelled 'Don't Use'
A number of facilities in Arlington recorded violations during recent inspections from the Arlington County Public Health Department.
ARLINGTON, VA — A number of facilities in Arlington recorded violations during recent inspections from the Arlington County Public Health Department. Inspectors conducted the most recent visits between March 19-20.
Inspectors noted that a kitchen in one Arlington restaurant had a severely dented tomato juice can that was labelled "Don't Use." The staff was told to return or discard any dented food cans.
"Food packages must be in good condition to protect the integrity of the contents so that food is not exposed to adulteration or potential contaminants," the inspector wrote in their report.

In Virginia, all food service locations are supposed to be inspected at least once per year. Facilities with higher risk factors may be inspected as often as four times per year, according to the Virginia Department of Health.
Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected. Sometimes, the follow-up inspection only considers the most important "priority" violations.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.Depending on the results of the inspection, the health department can pursue several actions.
They can issue a permit for the restaurant to continue serving food, they can schedule a follow-up inspection to determine if violations have been rectified, or they can temporarily shut down an establishment until certain issues are solved.
The Virginia Department of Health only recommends restaurants have their permits suspended when "the continued operation of the food service presents a substantial and imminent threat to the public health."

- Severely dented tomato juice can; label "Don't Use," redeem for credit and discard. Corrective Actions: Discard or return food products once the packaging is damaged or is in bad condition. Food packages must be in good condition to protect the integrity of the contents so that food is not exposed to adulteration or potential contaminants.
- Bar dish machine sanitizer level at 10 ppm; don't use until 50-100 ppm chlorine. Corrective Actions: Immediately discontinue the use of the machine until it is operating in accordance with minimum requirements. Initiate manual wash, rinse, and sanitize method to properly clean and sanitize equipment and utensils. To minimize workload you can move to all single service utensils.
- Milk at server station "Magic Chef" refrigerator at 44°F; relocated to bar refrigerator; keep at or below 41°F. Corrective Actions: Relocate food to a refrigeration unit that is capable of maintaining food storage at 41°F or below.
- Tomato soup found improperly cooling in a large deep covered plastic container in the walk-in refrigerator along with cooked potatoes and carrots at 81°F six inch deep pans with plastic wrap.
- Corrective Actions: Cool foods by the following methods: 1. Placing food in shallow pans 2. Separating food into smaller thinner portions 3. Using rapid chill cooling equipment 4. Stirring the food in a container placed in an ice water bath 5. Using containers that facilitate heat transfer 6. Adding ice as an ingredient 7. Any other method that can effectively accomplish cooling.
- Air gap at bottom middle of front entrance doors and under the meat walk-in refrigerator door. Corrective Actions: Protect the food establishment against the entry of insects and rodents by: 1. Filling or closing holes and other gaps along floors, walls, and ceilings; 2. Closed, tight fitting windows; and 3. Solid, self-closing, tight-fitting doors. Insects and rodents are vectors of disease-causing microorganisms which may be transmitted to humans by contamination of food and food-contact surfaces.
- Little flies observed in bar closet. Oriental dead roach observed under pizza kitchen hand sink. Mouse droppings observed on 2nd floor dining area closet. Corrective Actions: Eliminate harborage conditions. Insects and other pests are capable of transmitting disease to man by contaminating food and food-contact surfaces.
- Bar & server station upstairs scoop handles observed on ice. Corrective Actions: Store in-use utensils or dispensing utensils in one of the following manners: 1) In the food with their handles above the top of the food and the container. 2) In food that is not potentially hazardous with their handles above the top of the food within containers or equipment that can be closed. 3) On a clean portion of the food preparation table or cooking equipment only if the in-use utensil and the food-contact surface of the food preparation table or cooking equipment are cleaned and sanitized every 4 hours. 4) In running water of sufficient velocity to flush particulates to the drain. 5) In a clean, protected location if the utensils are used only with a food that is not potentially hazardous. 6) In a container of water if the water is maintained at a temperature of at least 135°F and the container.
- Bag of pizza boxes and little plastic cups & lids observed stored on the floor in staircase & upstairs closet; elevate onto shelving six inches above the floor; elevate items six inches off floor in the outside storage room. Corrective Actions: Discontinue storage of clean equipment and utensils in a location subject to contamination.
- Cloths used under cutting boards; use cutting board mats. Corrective Actions: Nonfood contact equipment that is exposed to splash, spillage, food soiling, or requires frequent cleaning must be smooth, durable, and nonabsorbent.
- Some cutting boards aren't kept clean to sight and the ice machine interior lid is damaged. Corrective Actions: Repair or replace the cutting boards and ice machine lid to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability.
- Inside the pizza kitchen 2-door counter and pizza prep refrigerators aren't kept clean. Corrective Actions: Maintain non-food-contact surfaces of equipment clean.
- Damaged atmospheric vacuum breaker at kitchen dish machine. Corrective Actions: Plumbing systems and components shall be maintained in good repair.
- Middle outside trash dumpster drain plug is missing. Corrective Actions: Replace the drain plug to the refuse container.
- Floors not kept clean in the pizza kitchen and outside storage room. Wall not kept clean behind the kitchen dish machine. Ceiling not kept clean in upstairs kitchen dessert prep area. Mold ant men's restroom sink vanity. Septic odor in rest-room hallway. Corrective Actions: All floors, walls, and ceilings must be cleaned as often as necessary to keep them clean. Cleaning of the physical facilities is an important measure in ensuring the protection and sanitary preparation of food. A regular cleaning schedule should be established and followed to maintain the facility in a clean and sanitary manner.

Corrective Actions: Ensure all food employees clean their hands and exposed portions of their arms with a cleaning compound in a lavatory by vigorously rubbing together the surfaces of their lathered hands and warms for at least 20 seconds and thoroughly rinsing with clean water. Observed mushrooms holding in prep table drawers at 71F, per person in charge, mushrooms had started cooling at around 11am. Corrective Actions: Cool time/temperature control for safety (TCS) food(s) from 135°Fto 41°F within a total of 6 hours and from 135°F to 70°F within 2 hours. A longer time for cooling allows an ideal situation for bacterial growth. This has been shown to be the major contributing factor in many foodborne illnesses. Corrected by reheating to 193 during inspection, and starting cooling again.
Observed that coleslaw, tomatoes and pasta in reach in cooler were not holding at 41F or below. Corrective Actions: Cold hold potentially hazardous food at 41°F or below to inhibit the growth of harmful bacteria. Adjust reach in cooler, and if foods do not reach 41F or below, relocate food to a refrigeration unit that is capable of maintaining food storage at 41°F or below.
Observed that walls are in need of cleaning in the grill and kitchen areas. Corrective Actions: Clean the facilities frequently to prevent the buildup of debris on floors and walls

Identification of these containers with the common name of the material helps prevent the dangerous misuse of the contents.
