Manassas Health Inspections: 24 Violations, Roaches At 1 Restaurant

Several restaurants around the Manassas area recorded violations during recent inspections from the county health department.

MANASSAS, VA — Several facilities in the Manassas area recorded violations during recent health department inspections between Oct. 11 and Oct. 17.

COVID-19 restrictions have mostly disappeared around the state. Health inspectors conduct most restaurant and other health inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.

Violations typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection. Other violations are addressed later, and an inspector may conduct a follow-up inspection to ensure compliance.

  • Coles Little League Snack Bar, 14420 Bristow Rd. Manassas, VA 20112
    • Date inspected: Oct. 11
    • Total violations: 11
      • There was no verifiable employee health policy was not available at the time of the inspection.
      • There was no procedure available during the inspection to show how to clean vomit and diarrhea.
      • An employee donned gloves before washing hands to engage with food prep.
      • There was no hand sink in the facility that was designated for hand washing only.
      • The 2 compartment sink was not set up properly when asked to do so.
      • Temped hot dogs in the hot holding cabinet at 101F, 103F, 117F.
      • In reach-in sliding door cooler, reach-in 1 door freezer and a hot holding cabinet there is no ambient air thermometer inside.
      • There was no thin probe thermometer available to take temperature of food items.
      • There were boxes of hotdog buns, chips and other snacks on the floor of the kitchen.
      • An employee was observed working with food without a proper hair restraint on.
      • There were no test strips for the chlorine chemical sanitizer solution.
  • Nico's Cantina & Grill, 120 Kent Village Manassas Park, VA 20111
    • Date inspected: Oct. 11
    • Total violations: 24
      • Employees are not aware of or are not practicing proper methods to rapidly cool potentially hazardous food (PHF).
      • Poor handwashing procedures observed. Employee washing hands in the 3-compartment sink.
      • Food employees are not informed of their responsibility to report to the person in charge information concerning their health and activities as they relate to diseases transmitted through food.
      • A food employee was observed cleaning their hands in a sink (3 compartments sink) that is not designated as a handwashing sink.
      • The packaged white soft cheese is not labeled as specified in law. No invoice available unable to determine source.
      • There is no written agreement or statement from the supplier or aquaculturist stipulating that the raw tilapia for ceviche, Raw, raw-marinated, partially cooked, or marinated-partially cooked fish@ served or sold in ready-to-eat form, is aquacultured and/or the records are not retained in the food establishment for 90 calendar days beyond the time of service or sale of the fish.
      • Raw chicken container stored on the above shelf of the asparagus container in the reach in cooler bottom. Raw animal food holding in a manner that may cause cross contamination of to ready-to-eat food (RTE).
      • Uncovered food tomatoes sauce in the upright reach in cooler. Opened bags of rice and beans in the dry storage area.
      • Surfaces of the cutting boards are not clean.
      • The prepared ready-to-eat (RTE) salsa prep on 10/4 and beef cooked on 10/2 were held more than 7 days in the refrigerator.
      • Unknown tracking powders being used in the food establishments floors and corners.
      • Insect spray bottle, poisonous or toxic materials found stored on the kitchen shelving.
      • The methods used for cooling tomatoes and peppers sauce were not adequate, covering containers.
      • Outer opening of the food establishment is not protected against entry of insects and rodents.
      • Observed live roaches and flies and death [sic] bugs on the premises of the food establishment.
      • Linens or napkins found in contact with food shredded lettuces.
      • Raw shrimp stored in direct contact with ice.
      • Employees observed working in the food service area without proper hair restraints.
      • In-use utensils improperly stored between use. Ice scoops store on top of the counter next to the ice machine
      • Spatulas (grill area) were observed in a state of repair and condition preventing the equipment to be used as designed.
      • The mop faucet vacuum breaker is under pressure a Y connection was installed to attach the mix chemical dispenser.
      • There is no refuse container at the area immediately adjacent to the all kitchen and food preparation handwashing sink areas.
      • Walk in freezer light fixtures are not working (no light in the freezer) is not maintained in good repair.
      • Less than 10 foot candles of light (no light "0 f/c) was noted in the Walk in freezer.
  • Domino's Pizza #4380, 8813 Centreville Rd Manassas, VA 20110
    • Date inspected: Oct. 12
    • Total violations: 1
      • The Person In Charge (PIC) was not able to provide documentation that all food employees or conditional employees were trained and informed of their responsibilities to report symptoms or diagnosis of, or exposure to, diseases transmissible through food.
  • Sabor Latino Restaurant Las Mejores Mangonadas, 9774 Zimbro Ave Manassas, VA 20110
    • Date inspected: Oct. 12
    • Total violations: 4
      • There is no person in charge with a valid Food Protection Manager Certification from an accredited agency recognized by the Conference for Food Protection.
      • The Person In Charge (PIC) was not able to provide documentation that all food employees or conditional employees were trained and informed of their responsibilities to report symptoms or diagnosis of, or exposure to, diseases transmissible through food.
      • The food establishment does not have written procedures for cleaning up vomiting or diarrheal events that happen in the facility.
      • Raw chicken stored over raw fish in walk in cooler.
