Observations: Failure of Person in Charge to monitor and ensure proper food safety handling taking place in facility. Person in Charge is to monitor employees to ensure good food handling practices are taking place within facility. Five (5) containers of washed and prepped onions were stored in walk-in cooler uncovered. Chemical sanitizer (quat ammonium) in 3-compartment sink below approved concentration of 200-400 ppm while dishes were being cleaned. Knife sheaths for hibachi chefs and inside of microwave contain food debris/residue buildup. Cooked potentially hazardous foods not cooled from 135F to 41F within a total of six (6) hours. Food discarded. Fish oil and other dietary supplement tablets stored on shelf above food prep station and fruit/vegetable sink. Potentially hazardous foods (chicken soup and seafood sauce(contains cream and butter)) cooling in deep, large containers with tight-fitting lids. Container of onions stored on floor in walk-in cooler. Zuchinni and mushrooms, already washed and cut by facility, stored in boxes that food had been delivered to facility. Food rewashed and placed in plastic containers. Storage shelves in kitchen contain grease and dust buildup. Walls and ceilings contain dust buildup. Back door kept open while not in use. Door was open at beginning of inspection.

A food dispensing utensil shall be available for each container displayed at a consumer self-service unit such as a buffet or salad bar. **The utensil’s length shall be longer than the widest portion of the container.**

Squeeze bottle of sauces and bulk sugar not labeled with common name. Paper towels used in direct contact with cilantro. Pans observed being stacked wet. Single use containers stored on the floor in outside storage room. Observed employee going from raw meat to handling equipment that will also be handled before touching ready to eat foods. Doors of dumpster open during inspection.

Food encrusted in the microwave. Missing and/or worn consumer advisory labels on the menus. Sanitizer buckets stored on cookline shelves, next to food & utensils. Broken wires on fry baskets; some wire pieces are missing - contamination risk. Observed some containers with stickers or sticker residue remaining on the exterior surface of the container. Heat lamp bulbs are not shatter-resistant.
