Remarks: Unwashed avocados stored above cut/washed/ready-to-eat green onions on the rolling rack. Observed cut onions in the walkin cooler that were not washed prior to cutting.

Remarks: CFSM left in August has not been replaced. Facility must a have a valid CFSM on staff. lasagna and beef past 7 days. Items discarded. All foods that were date marked were date marked with 8 days instead of 7 days. In use utensils stored in room temperature water. Utensils sent to dishwashing. No hot water at the handsink in the kitchen area. Must have hot water under pressure available at all handsinks at all times.

Chlorine >200PPM in two sanitizer solution buckets for cloth towels. Solution diluted to 100ppm chlorine. Spray bottle with chemical not labeled. Had facility label chemical. Dishes stacked wet. Exterior of food storage containers with old glue and residue build up.

Salsas, corn, pico, cut tomatoes did not cool to 41 deg. F within 4 hrs. Foods covered & not stored in coldest area of cooler with adequate air flow. Product DISCARDED. Employees working with exposed food wearing watches & bracelets. Wet towels in cubicles. Cloth towels must be stored in sanitizer between uses. Food service permit not posted.

Raw meat in direct contact with warewashing sink surface. Screen door not self-closing. gap along bottom of screen door.

Remarks: Interior of ice machine around ice shoot with slime-like build-up. Tables cleaned one time with towel and detergent. Reviewed how to properly clean tables with staff. Re-trained employee on how to properly clean tables. Food storage stacked wet. Employee working with food wearing glove that was torn, ripped. Had employee remove damaged glove(s) and wash hands. Food storage containers chipped/cracked. Stickers and old glue residue still present on exterior surface of food storage containers.
