The annual bookbag/school supply giveaway and free medical screenings will be held on July 18 from 9am to 1pm at Hiram Elementary School.

To register, please email the following information to Gary Davis, at garydavis1058@yahoo.com no later than Thursday, July 16:

1) Parent’s Name: Last, First (Your child(ren) will be registered under your LAST name)
2) Child(ren)’s name(s)
3) Child(ren)’s grade level(s)
4) Parent’s phone number
5) Parent’s email address, if available
6) Indicate your preferred appointment time of 9am, 10am or 11am

This program is coordinated by Gary Davis of Outreach Ministries and supported by the City of Hiram, with the assistance of community volunteers and school supply donations by our local Hiram WalMart.
