Remarks: A photocopy of an owner’s food safety certificate is on display. Remove the duplicate of the owner’s certificate. Manager failed to notify the health department within 30 days of the expiration of her food safety certificate. Manager is taking the food safety exam today. Employee was observed drinking water and eating cookies on counter near the convection oven. Employee beverage was seen on the cutting board of the vegetable sliceer. The food and beverages were removed. Quaternary ammonium sanitizer for pans and other wares was less than 150 ppm in concentration. Boxes and bags, containing Subway garbage, were scattered on the ground behind the dumpster. Lightbulb shield above the bread rack is unclean.

Remarks: Whole milk, which expired on 10/28/2014, was still in use. Quaternary ammonium sanitizer was being used at 0 ppm. Manager prepared a new solution and tested it. Quat sanitizer must be maintained at 200 ppm . Cutting board is deeply scored and has black discoloration. A screw, which supports the ice shield, inside the automatic ice machine is rusted; a trail of rust is on the wall inside the ice machine. Both doors of the upstairs restrooms are propped open. Garbage bags were thrown behind the dumpster. Multiple acoustic tiles in toilet rooms were observed with moisture stains from water damage. Replace all of the acoustic tiles with solid, washable vinyl tiles. Ventilation fan in the upstairs men’s restroom is not functional.
