Onion straws (made in house) stored at room temperature not documented with the time when removed from temperature control. Had facility discard wings. Onions not documented properly with the time when removed from temperature control as per written procedure. Three spray bottles of chemical not labeled. Employees wearing band, watch, etc. on wrist while working with food. Jewelry: Except for a plain ring such as a wedding band, while preparing food, food employees may not wear jewelry including medical information jewelry on their arms and hands. Multiple used cloth towels on countertops. Had facility relocate towels to sanitizer solution. Personal food, cigarettes stored in kitchen next to food equipment/food.

Some ceiling tiles in kitchen water damaged or replaced with cardboard.

A food dispensing utensil shall be available for each container displayed at a consumer self-service unit such as a buffet or salad bar. **The utensil’s length shall be longer than the widest portion of the container.**
