Employees wearing band, watch, etc. on wrist while working with food. Jewelry: Except for a plain ring such as a wedding band, while preparing food, food employees may not wear jewelry including medical information jewelry on their arms and hands. Multiple used cloth towels on countertops. Had facility relocate towels to sanitizer solution. Personal food, cigarettes stored in kitchen next to food equipment/food.

Some ceiling tiles in kitchen water damaged or replaced with cardboard.
