Translate Monday, November 29 at Eastside Elementary Tuesday, November 30 at Chestnut Log Middle Wednesday, December 1 at New Manchester Elementary Thursday, December 2 at Bill Arp Elementary and Friday, December 3 at Winston Elementary For children’s vaccinations, we will require . Please be prepared to provide the last four digits of the child’s social security number or a copy of the parent’s insurance.   Please bring your current CDC vaccine card if coming for a second, third, or booster dose.   This vaccine event is a community-wide event. It will be open to the public, including adults and students from other schools.
