Remarks: Observed employee pick debris off the floor with her hands and continue to work with equipment without washing her hands. Observed employee leave the prep area, return, touch her face, and handle clean equipment and then put on gloves without washing hands first. Observed employee wash the rice cooker drum and lid and place them back into the cooker without sanitizing before. Observed one fluid lighter stored on the chefβs cart over food. Observed another stored on a shelf above plates. Chlorine sanitizer used for wet wiping cloths was observed with a concentration above 100 ppm. The inspection report posted was 5/1/14 97A. The most current inspection report shall be prominently displayed in public view at all times. Utensils were observed being stored in two containers of water that was at room temperature. Observed the ice scoop used to place ice in drinks with the handle in direct contact of the food. Observed waste water over flowing from the floor drain near the only handwashing sink. Also, the drain near the wok was noticed with an elevated level of sewage that increased when the handwashing sink was used. The drainage system must be serviced to ensure that waste is properly removed from the establishment. Sewage must be conveyed to the point of disposal through an approved sanitary sewage system or other system, including use of sewage transport vehicles, waste retention tanks, pumps, pipes, hoses, and connections that are constructed, maintained, and operated according to law.
NOTE: The food service permit has been SUSPENDED due to violation Item 16C [code provision .06(4)(d),(e)]. All food service must cease until the issue is resolved. Please contact the Gwinnett County Environmental Health Department with notification of correction. At that point, an informal inspection will occur to ensure the issue has been resolved.

Remarks: Observed an employee return to the prep area and place candy/food in mouth. Dishes, table tops, and prep surfaces were being sanitized in quaternary ammonia that had a temperature below 75Β°F. Observed an employee wipe down several dining tables with a single wet wiping cloth. The cloth were never returned to the solution between any of the tables. Sanitizers buckets were observed stored in direct contact of the floor throughout the facility. Plates for salad prep and plastic containers (in clean storage) were observed stacked wet. The facility does not have hot water supplied to it (water temperature is 62Β°F). The facility must have the hot water heater repaired. The facility closed until the issue is resolved. Hot water generation and distribution systems must be sufficient to meet the peak hot water demands throughout the food service establishment.

Remarks: The person-in-charge was not able to convey the reportable symptoms and illness from the facility to the health department per the GA Food Code. Observed an contain of yogurt stored in ice used for beverages (iced coffee drinks, etc) to keep the yogurt cold. The food was removed, and the person-in-charge was informed. The handwashing sink behind the food prep line does not have a trash can nearby. Part of the door frame to the menβs stall is missing at the base--therefore, a hole is present; no covering.

Remarks: Observed employee handle dirty dishes at ware washing station then proceed to handle clean dishes without washing hands. Observed an employee handle lettuce from prep cooler with bare hands. Lettuce that was handled was discarded and employee washed hands and put on gloves. Observed employee handle shredded cheese with bare hands. Cheese that was handled was discarded. Lemons at drink station located in dining room display in a container that continuously exposes them to contamination. Lemons were moved from drink station. Observed employee working with food wearing a gold bracelet. The most current inspection report was not posted. Todayβs inspection report will be posted. Shelving in the back kitchen area where clean dishes are being stored has an accumulation of soap scum and dirt.

Remarks: Hand sink was blocked by caution sign and mop bucket in kitchen area. Sign and bucket were moved. Hand sinks need to be accessible at all times so that employees can wash their hands frequently. Cream cheese and waffle batter were above 41F. Sausage gravy at buffet was below 135F. Facility did not have digital thermometer capable of measuring the temperature of thin masses of food. Condiments for oatmeal not sufficiently protected from sneeze guard.
